TUJUAN PRAKTIKUM 1. Mahasiswa dapat menyusun rancangan formula, pembuatan, evaluasi dan kemasan gel serta mendiskusikan berdasarkan karakteristik fisika kimianya. 2. Mahasiswa dapat membuat sediaan gel yang telah dirancang dan mengevaluasi sediaan yang telah dibuat.

DASAR TEORI 1. Anatomi dan Fisiologi Kulit Kulit merupakan lapisan pelindung tubuh yang sempurna terhadap pengaruh luar, baik pengaruh fisik maupun pengaruh pengaruh kima. Dimana kulit berfungsi sebagai sistem epitel pada tubuh untuk menjaga kelurnya substansi-substansi penting dalam tubuh. Meskipun kulit relatif permeabel terhadap senyawa kimia namun dalam keadaan tertentu kulit dapat ditembus oleh senyawa senyawa kimia namun dalam keadaan tertentu kulit dapat ditembus oleh senyawa senyawa obat/bahan yang berbahaya yang dapat menimbulkan efek terapetik / efek toksik baik yang bersifat setempat/.sistemik. 2. Absorbsi Perkutan Absorbsi perkutan adalah masuknya molekul obat dari kulit ke dalam jaringan dibawah kulit kemudian masuk ke dalam sirkulasi darah dengan mekanisme difusi pasif. Istilah perkutan menunjukan bahwa penembusan terjadi pada lapisan epidermis dan penyerapan dapat terjadi pada lapisan epidermis yang berbeda beda.(Aiache, 1993). Penentuan molekul dari bagian luar ke bagian dalam kulit secara

dapat
terjadi
baik
melalui
penetrasi
transpidermal
dan
transpendegeal(Swarbick dan Boylan. 1995). Untuk memasuki sistem sistemik, tahapan pada absorpsi perkutan dapat melalui penetrasi pada permukaan stratum corneum di bawah gradien konsentrasi, difusi melalui stratum corneum, corneum, epidermis dan dermis, kemudian masuknya molekul ke dalam mikrosirkulasi (Aiache.1993) (Ansel. 2008) Tahapan ini dapat digambarkan pada gambar 2.

Pada prinsipnya masuknya penetran ke dalam stratum korneum adalah adanya koefisien partisi dari penetran obat – obatan obatan yang bersifat hidrofilik akan berpartisi melalui jalur transseluler sedangkan obat – obat obat yang bersifat lipofilik akan masuk kedalam stratum korneum melalui intraseluler (Swarbick dan Boylan. 1995). b. Penetrasi Transapendegeal Penetrasi melalui rute transapendegeal adalah penetrasi melalui kelenjar folikel yang ada pada kulit. Dimana penetrasi transapendegeal akan membawa senyawa obat melalui kelenjar keringat dan kelenjar rambut yang berhubungan dengan kelenjar sabapeus. Pada rute ini, dapat menghasilkan difusi yang cepat dan segera setelah penggunaan obat karena dapat menghilangkan waktu yang diperlukan obat untuk melewati stratum korneum (Swarbrick et al, 1995). 3. Definisi Gel Gel merupakan sistem semipadat yang terdiri dari suspensi yang dibuat dari partikel anorganik yang kecil atau molekul organik yang besar, terpenetrasi oleh suatu cairan (Departemen Kesehatan RI, 1995). Gel pada umumnya memiliki karakteristik yaitu strukturnya yang kaku. Gel dapat berupa sediaan yang jernih atau buram, polar, atau non polar, dan hidroalkoholik tergantung konstituennya. Gel biasanya terdiri dari gom alami (tragacanth, guar, atau xanthan), x anthan), bahan semisintetis semisintet is (misal : methylcellulose, carboxymethylcellulose, atau hydroxyethylcellulose), bahan sintetis (misal : carbomer), atau clay (misal : silikat). Viskositas gel pada umumnya sebanding dengan jumlah dan berat molekul bahan pengental yang ditambahkan. Gel dapat dikelompokkan menjadi : lipophilic gels dan hydrophilic gels. Lipophilic gels(oleogel) merupakan gel dengan basis yang terdiri dari parafin cair, polietilen atau minyak min yak lemak yang ditambah dengan silika koloid atau sabun-sabun sabun-s abun aluminium atau seng. Sedangkan hydrophylic gels, basisnya terbuat dari air, gliserol atau propilen glikol, yang ditambah gelling agent seperti amilum, turunan selulosa, carbomer dan magnesium-aluminum silikat (Gaur et al, 2008). Berdasarkan sifat pelarut terdiri dari hidrogel, organogel, dan xerogel. Hydrogel (sering disebut juga aquagel)merupakan bentuk jaringan tiga dimensi dari rantai polimer hidrofilik yang tidak larut dalam air tapi dapat mengembang di

cairan organic yang tertahan dalam jaringan cross-linked tiga dimensi. Cairan dapat berupa pelarut organic, minyak mineral, atau minyak minyak sayur. Xerogel berbentuk gel padat yang dikeringkan dengan cara penyusutan. Xerogel biasanya mempertahankan porositas yang tinggi (25%),luas permukaan yang besar (150-900 m/g), dan ukuran porinya kecil (1-10 nm). Saat pelarutnya dihilangkan di bawah kondisi superkritikal, jaringannya tidak menyusut dan porous, dan terbentuk aerogel. Gelling agent bersifat hidrofilik dan larut dalam air. Gom alam dan polimer berfungsi dengan membentuk lapisan tipis pada permukaan partikel. Pada saat dikempa, partikel cenderung beraglomerasi. Bahan sangat larut seperti gula, mengikat partikel bersama dengan membentuk jembatan kristal. Pengikat untuk proses granulasi basah biasanya dilarutka dalam air atau suatu pelarut biasanya berupa alkohol dan larutan pengikat digunakan untuk membentuk masa basah/granul. Dalam pengikatan partikel parti kel bersama yang berperan adalah ikatan van der walls dan ikatan hidrogen. Contoh : mikrokristalin selulosa, gom arab. Penggunaan gelling agent dengan konsentrasi yang tinggi mengakibatkan viskositas dari gel meningkat pula sehingga bisa mengakibatkan gel akan sulit dikeluarkan dari wadahnya. Temperature yang tinggi pada saat penyimpanan akan mengakibatkan konsistensi dari basis berubah, misalnya pada hydrogel yang sebagian besar solvennya berupa air maka temperature yang tinggi akan mengakibatkan

mengakibatkan perubahan pada struktur gel.Basis gel sebagian besar berupa polimer – polimer. Gel merupakan crosslinked system dimana aliran tidak akan terjadi apabila berada dalam keadaan steady state. Sebagian besar bahan merupakn liquid tetapi gel memiliki sifat seperti padatan karena adanya ikatan 3 dimensi didalam larutan. Ikatan ini mengakibatkan adanya sifat swelling dan elastic. Untuk melihat kerusakan dari struktur gel dapat dilihat dari kekakuan/rigidness dari gel tersebut. Temperature tinggi dapat mengakibatkan kekakuan dari gel meningkat oleh karena itu proses penyimpanan dari sediaan bentuk gel harus diperhatikan. 4. osteoarthritis ditunjukkan bahwa AINS baik yang non selektif maupun yang selektif menghambat aktifitas COX-2 berkhasiat dalam mengurangi nyeri rematik (Kumar, 2011) Makin lebih selektif suatu AINS menghambat COX-1 makin berkurang khasiatnya sebagai antiinflamasi dan sebaliknya dengan sediaan yang makin lebih selektif menghambat COX-2. Penggunaan COX-2 sebagai obat analgetika tunggal akan menunjukkan efek mengatap. Waktu paruh dalam plasma lebih dari 45 jam sehingga dapat diberikan hanya sekali sehari. Absorbs berlangsung cepat dilambung, terikat 99% pada protein plasma. Kira – kira kira sama dengan kadar cairan sinovia. Efek samping tersering adalah gangguan saluran cerna, antara lain yang berat adalah tukak lambung. Efek Ef ek samping tersering adalah pusing, tinnitus, nyeri kepala dan eritema kulit. Piroksikam tidak dianjurkan diberikan pada wanita hamil, pasien tokak lambung dan pasien yang sedang minum antikoagulan indikasi piroksikam hanya untuk penyakit inflamasi sendi misalnya arthritis momatoid, osteoarthritis, sponditis ankilosa. Dosis 10-20 mg sehari (Syarif. 2007) Piroksikam merupakan antiinflamasi non steroid (AINS) emmpunyai sifat tidak larut dalam air, asam-asam encer dan sebagian besar pelarut organic, sehingga perlu diupayakan untuk menaikkan kelarutannya dengan penambahan surfaktan (Kumar, 2011). Prinsip kelarutan piroksikam adalah stabilitas yang sangat baik pada pH 7,5 dengan pKa 6,3. Factor yang mempengaruhi laju degradasi antara lain pH, dapar, suhu, media reaksi dan adanya bahan tambahan seperti surfaktan (Kumar, 2011) Pada penelitian ini bentuk sediaan terpilih adalah gel mempunyai kadar air yang tinggi sehingga dapat mengurangi kondisi panas dan tegang yang sifatnya setempat dan timbulnya kulit meradang. Gel diaplikasikan langsung pada kulit yang mengalami gangguan dan setelah kering akan meninggalkan lapisan tipis tembus pandang, elastic dengan daya lekat tinggi,
