Fistel uretrokutan dan Penanganannya Penanganannya
Hipospadia merupakan salah satu dari kelainan kongenital pada bayi yang paling sering, terjadi berkisar antara 3 sampai 5 per 1000 kelahiran hidup.
Komplikasi awal dari repair hipospadia, adalah termasuk perdarahan, hematoma, dan retensi urin
Komplikasi lambatnya adalah stenosis meatal, fistula enterokutan, divertikula uretra, kordae yang berulang dan striktur uretra
Fistula uretrokutan pascarepair hipospadia, merupakan komplikasi yang paling sering ditemukan, dengan angka rata-rata kejadian yang pernah dilaporkan sekitar 4-25%
INSIDENSI FISTULA URETROKUTAN PASCAOPERASI HIPOSPADIA
Angka kejadian terjadinya fistel uretrokutan pascaoperasi hipospadia, bervariasi, antara 0 – 23%, tergantung pada masing-masing tempat penelitian, bahkan ada yang menyatakan angka kejadian ini berkisar antara 0 – 45%, tergantung pada spektrum hipospadia yang diteliti, teknik operasi dan lamanya masa penelitian
Dari kisaran ini, rata-rata komplikasi fistula uretrokutan, terjadi antara 6 bulan sampai 12 bulan setelah operasi hipospadia yang pertama, tetapi ada laporan bahwa kejadian komplikasi ini terjadi 12 tahun setelah operasi pertama.
Sekitar 50% sampai 70%, kejadian fistula ini, te rdapat pada hipospadia letak medial dan posterior
Srivastava RK, dkk, dari Saraswati Hospital and Research Cente r, Lucknow, India, meneliti bahwa midpenile, merupakan lokasi yang paling sering terjadinya komplikasi fistel uretrokutan pascaoperasi hipospadia (30% dari keseluruhan kasus)
PENATALAKSANAAN
Penutupan fistula dengan jahitan simpel, mudah dilakukan dan tidak membutuhkan waktu yang banyak, namun jika dilakukan diatas jahitan sebelumnya, merupakan hal yang potensial untuk terjadinya rekurensi.
Flap kulit biasa digunakan untuk memperbaiki fistula yang besar bagi penutupan simpel, dan membuat kulit lokalnya lebih lembut dan adekuat
Algoritma penanganan fistula uretrokutan setelah repair hipospadia, sesuai dengan ukuran dan lokasi dari fistula
Van der Meulen, et al , menyatakan ada enam (6) faktor yang menyumbang kejadian fistula berulang, yaitu: •
(1) devaskularisasi pada kulit akibat tidak adekuatnya delineasi pada flap kulit atau penarikan kulit akibat dressing yang ketat,
•
(2) tarikan yang kuat pada kulit akibat kombinasi dari sedikitnya kulit yang tersisa dan edema sekitar luka,
•
(3) superposisi dari uretra dan garis jahitan kulit,
•
(4) infeksi pada luka, yang menyebabkan devaskularisasi pada kulit atau stagnansi (berkumpulnya) darah dan urin ISK
•
(5) perforasi pada kulit akibat jahitan transkutaneus, dan
•
(6) terpisahnya tepi luka akibat tidak adekuatnya aliran urin
Penelitian yang dilakukan oleh Meir DB, dkk, menyatakan bahwa penggunaan antibiotika spektrum luas, secara signifikan, pre dan pascaoperasi hipospadia, sangat dibutuhkan untuk
menghindarkan terjadinya bakteriuria yang menyumbang faktor terjadinya komplikasi fistel pascaoperasi hipospadia, sampai dengan 14 hari pascaoperasi hipospadia atau repair fistel
Terbentuknya fistula uretrokutan pascaoperasi hipospadia, masih menyisakan sesuatu yang membuat putus asa, bagi pasien dan para ahli bedahnya. Walaupun sudah dilakukan evaluasi dan perencanaan preoperasi, teknik bedah yang banyak dan baik, penggunaan optikal, asisten yang terlatih dan alat-alat bedah yang moderen.

Walau sudah terdapat algoritma penanganan fistel uretrokutan, tetapi komplikasi yang terjadi pascarepair fistel uretrokutan, masih tetap terjadi, dan menyisakan tantangan bagi para ahli bedah anak, untuk tetap mengembangkan teknik-teknik terbaru, disesuaikan dengan pengenalan anatomis dan patofisiologi yang lebih mendalam
Fistula : Fistula uretrokutan merupakan masalah utama yang sering muncul pada operasi hpospadia.Fistula jarang menutup spontan dan dapat diperbaiki dengna pe nutupan berlapis dari flap kulit lokal

The application of scrotal dartos or TVF for wrapping the repaired area gives excellent results and they are easy to harvest with no harmful effects on the sc rotum or testis.

Intraoperative calibration of the distal urethra and distension of the repaired hypospadias to search for a diverticulum are recommended. Coronal fistulas are best repaired by converting them into coronal hypospadias, followed by tubularization of the urethral plate with or without a dorsal midline relaxing incision. In resurfacing the operative site, the traditional transposition flaps (Y-V and advancement) may be unreliable, because their vascularity may be compromised by previous surgery. The hairless scrotal island or rotation scrotal flap is more reliable for these cases. T. Cass Urethrocutaneous fistulas complicating hypospadias repair appear a common problem. There appears less data in the literature r egarding the risk and management of recurrent fistulas. A retrospective review of urethrocutaneous fistulas complicating hypospadias repair was performed to evaluate their aetiology, management and outcome. Betwee n 1993 and 2003, 1,753 patients had a hypospadias repair at our institution. Overall 123 (7%) boys deve loped a fistula, although detailed information was available on 117 patients only. Median age was 3.5 years at the time of fistula repair; 13% had anterior, 57% had middle and 30% had posterior hypospadias. The most common primary surgical procedure was a Durham Smith two-stage repair in 29% (n = 34), followed by a Tubularised Incised Plate urethroplasty in 19% (n = 22 ) and an Onlay in 14% (n = 16). Thirty-one (27%) patients developed a recurrent fistula, 9 (29%) of which recurred following a second repair. None recurred after a third repair. The risk of a recurrent fistula after an initial distal fistula repair was 12 .5% and after a posterior fistula was 62% (χ2 = 15.4, P = 0.001). Use of a stent, suture type, numbers of fistula and closure attempts
did not influence the recurrence r ate. Undiagnosed distal obstruction was thought to be related to 27 of 117 first fistula repairs (23%) and 4 of 31 second fistula re pairs (13%). The risk of recurrent urethrocutaneous fistula was increased in those boys with a posterior fistula, following a simple repair or when there was evidence of distal urethral obstruction.

Methodology: This was a prospective study conducted in department of urology Jinnah Postgraduate Medical Centre (JPMC), Karachi. Seventeen case s with urethrocutaneous fistulas were enrolled in this study. In 12 (75%) we did the simple repair and second layer w ith dartos, in 03 (16.7%) OIU and silicone catheterization was done, but the y recurred then repair with tunica vaginalis cover was performed and 02 (8.3%) patient underwent OIU plus simple repair with dartos. Results: The mean age was 25.58 � 6.2 years, in 11 post hypospadiasis was the cause of urethrocutaneous fistula. Recurrence was observed in four patients. Two patients afte r post hypospadias repair; one after stricture peno-scrotal and one after post infection, boil at base of penis had recurrence. Two patients with simple repair and 2 nd layer with dartos had recurrence, two patients treated with OIU had recurrence and after that repair with tunica vaginalis cover was performed with no recurrence and none of the patients treated with OI U plus simple repair with dartos had recurrence. Conclusions: Circumferential incision around the fistula opening, tension free closure with fine PDS suture is the key for successful treatment of urethral fistula.
