4. Unsafe condition on the lifting gear (chain sling) the hook latches were missing.

Unsafe lifting operation. Foreman in charge assigned incompetent banks man to sling loads.

Monthly colour code inspection of the sling has not implemented.

5. Lack of information on the hazardous substance (Diesel Oil) used in carpentry area.


Instructed to remove the chain sling from service. Instructed to have a thorough inspection carries out in all the lifting gear prior to use.

1 month Training plan for the month this is to include banks man training, hence to ensure that the Lifting activities should be carried out only under the supervision of competent supervisor and banks man. This is to ensure that all lifting accessories are thoroughly examined by a competent person every 6 months. All lifting accessories that have been thoroughly examined by a competent person. All hazardous substances which are pose a risk to workers health( i.e. skin and eye irritation, etc.) must have a manufacturers safety data sheets (MSDS) available to be used as a guide in conducting risk

6. Overloaded dump trucks carrying sand and soil which may cause falling of rocks from the load and may hitting other vehicles nearby. Risk of vehicular accident/incident due to over loading.

7. Hand tools inspection is not in place. Since we found unsafe hand tools (loose hammer head, chisel with a mushroom head and makeshift hand tools at the carpentry area).

8. Wearing of loose cloths and jewellery’s while using portable tools such as grinder.

Caution sign must in place to warn people. Weather condition must observe. PPE must be wearing during work such as hard hat, cover all uniform, goggles, harness and gloves. Adequate supervision.
