EXECUTIVE SUMMARY Background Chronic pancreatitis (CP) is characterized by irreversible damage to the pancreas, development of histological evidence of inflammation and fibrosis, which eventually leads to destruction of the exocrine (acinar cell) and the endocrine (islets of Langerhans) tissue. Because many patients report of unexplained abdominal pain, the symptom eludes timely diagnosis of Chronic Pancreatitis. Hence, the prevalence of the disease remains unknown. It is interesting to note that India is home to a large population of patients suffering with Chronic Pancreatitis, especially the population from the southern part of the country. However, in western countries, in a population of 100, 000, generally, 8 to 10 cases are reported per year, which if compared to Indian population (114-200 cases per year) is a miniscule number. Of the many causes of Chronic Pancreatitis, alcohol consumption is majorly linked with this disease, except for Tropical Calcific Pancreatitis (TCP), a juvenile form of chronic calcific nonalcoholic pancreatitis and is exclusively reported in the developing countries of the tropical world. Tropical Calcific Pancreatitis is more prevalent and constitutes more than two-thirds of causes of Chronic Pancreatitis in India. And the mean age of onset of the disease is observed as 24 years -affecting more male than female population. The pathophysiology of tropical pancreatitis is unknown. Protein-calorie malnutrition, deficiency of trace elements and micronutrients, and dietary factors including cassava (tapioca) predispose to the disease. On the other hand, Chronic Pancreatitis is quite fatal in nature. Overall, 10-year survival is about 70%, and 20-year survival is about 45%. Besides posing threat of pancreatic cancer in 40 per cent of patients diagnosed with Chronic Pancreatitis, it may also cause other complications such as pseudocyst, common bile duct obstruction, fistulas and diabetes. Genesis of the herbo mineral formulation (HMF) Nearly four decades before Meerut based Vaidya Chandra Prakash formulated a complex compound using copper, mercury and sulphur following the guidelines laid down in Rasa Shastra. Coincidently, this formulation was administered to a terminally ill young patient of pancreatic cancer who miraculously recovered and enjoyed 38 years of disease free survival. Though this formulation worked for a few patients suffering with pancreatic cancer; it was found effective in treating patients of Chronic Pancreatitis. Eventually patient inflow increased from a few to many from 1997. To retrace the success of this treatment protocol, a detailed analysis of the patient data was done in 2014. It was found that out of 56 patients 32 treated using this herbo-mineral formulation completed over a year treatment and are leading a symptom free life. Step forward Based on the empirical success of herbo-mineral formulation in the treatment of Chronic Pancreatitis, the treatment protocol was discussed with scientists and policy makers in presence of beneficiaries’ patients in an event organized in November 2014 at Dehradun. Following the recommendations made by experts, Hon’ble Chief Minister of Uttarakhand, Shri Harish Rawat agreed to extend logistic and financial support towards the scientific development of this formulation under the aegis of Uttrakhand State Council of Science & Technology (UCOST). In lieu with the announcement, a project called “Development of Herbo Mineral Formulation (HMF) for Chronic Pancreatitis” was launched in January 2014. The proceedings of this event

EXECUTIVE SUMMARY received wide media attention and soon after the event, distressed patients and their families approached our Centre in Dehradun. In this report, we present first hundred cases those who registered for Ayurvedic treatment. Our 100th case, enrolled in March 2015, is 10 years old; his name is Karthik, a resident of Australia. Data Collection and Case Studies All these patients who approached us were diagnosed of CP by doctors of repute employed with world-class hospitals across the country. Modern diagnostic tools and parameters such as USG, ERCP, MRCP, EU and other were deployed to make accurate diagnosis. During the time of medical review by our team, it was found that most of these patients had either received conventional treatment for a year or more and were in a fragile physical and mental state. In this report, we present the data collected from 1997 till May 2015. • 58 patients enrolled during 1997 to 2014 • 42 patients enrolled during December 2014 to May 2015 A giant leap: on the way to scientific development Even though CP can be managed using conventional medical aid, it however, remains a huge challenge for the medical fraternity to treat it completely. Till date no known cure of CP has been reported. In such a scenario, the herbo-mineral formulation developed by VCP Cancer Research Foundation (VCPCRF) in the treatment of CP has been found sustainable with no side-effects. It will take approximately 7 years to near a logistic conclusion from the state sponsored project “Development of HMF in the treatment of CP”, a project based on the principles of reverse pharmacology. Meanwhile, protocol based Ayurvedic treatment is being made available to patients under close observation and long term follow-up as per the guidelines of good clinical practices. A preliminary analysis is being carried out at the Indian Institute of Science (IISc), Bangalore and IIT Roorkee to test if the end product turns into a compound of oxidized minerals and has no trace of free metal. During the first three years, the multi-step drug processing will documented using scientific tools like particle size analyzer to standardize the process from grinding to programmed muffle furnaces—to provide control heating and non-destructive analysis after each step—to understand the sequence of changes as well as to set up benchmark standards. These are all essential steps for reproducibility and future industrial production. In the second stage, safety studies (acute, sub-acute and chronic) will be carried out on rats and mice. In earlier studies, no Grade II toxicity (unpublished data) was reported. Herewith, a systematic safety study will be carried in collaboration with competent institutes. Finally, multicentric randomized controlled clinical trial will be carried at the State and National level to merge the benefits of traditional wisdom of Ayurveda into mainstream medicine. PREFACE Chronic disease is a long-lasting condition that can be controlled but not cured. Persistence of these diseases brings enormous financial and emotional stress to an individual and the whole family. Chronic Pancreatitis (CP) is one such emerging disease which is diagnosed with inflammation of the pancreas that does not heal or improve, worsen over time and leads to permanent damage. CP eventually, impairs ability to digest food and to make pancreatic hormones, and sometimes result in diabetes or pancreatic cancer. Incidence of CP in India has reached to an alarming situation where almost 114/100000 people in Southern part has been reported as compared to 3.5-10/100000 of developed countries. CP often develops in the most productive age group of 30-40 years, and is more common in men than women (4:1). The most miserable is the fact that there is no known cause and cure for CP in modern medicine. Patients are generally given pain killers, pancreatic enzymes or insulin for life long as preventive measure of severity of disease. However, a herbo-mineral formulation (HMF) prepared with Cu, Hg and S and some herbs by Vaidya Balendu Prakash has shown remarkable success for complete and sustainable treatment of CP. Vaidya Balendu claims that toxicological studies carried out on mice have shown its non-toxic nature and processing of metals following the principles of Ras-shastra indeed has a role to play in detoxifying the harmful effects of raw Cu, Hg and S. The intrigue chemistry of HMF largely remains unknown so far. Recently, the Government of Uttarakhand has taken note of this innovation and funded a seven year project, “Development of Herbo-Mineral Formulation for Chronic Pancreatitis” to Vaidya Ji. This project is being done following the approach of reverse pharmacology at VCP Cancer Research Foundation, Dehradun, under the aegis of Uttarakhand State Council of Science & Technology (UCOST). The evidence is required to prove an idea or concept or even a system. Council is of the opinion that Ayurvedic drug discovery system need to integrate with modern scientific tools and techniques for its evidence based global promotion. Robust documentation of clinical observations for Ayurvedic formulations and publication of clinical data need to be encouraged in order to make a strong foundation of Ayurvedic drug promotion. Curative effects of HMF in well diagnosed approximately 110 patients of CP have been observed and documented by Vaidya Ji for decades in his clinical practice. Council is extremely happy to learn that Vaidya Ji is publishing a booklet on his efforts towards treatment of chronic pancreatitis using HMF. Chronic Pancreatitis (CP) is an inflammatory disease of the pancreas with irreversible morphologic changes. It is characterized by moderate to severe pain in the upper abdomen radiating to middle of the back associated with nausea, vomiting, weakness and weight loss. Such symptoms might last from hours to two days and eventually could be continuous as the condition get worse. Inflammation of the pancreas that does not heal, worsens over time and leads to permanent damage. It occurs when digestive enzymes attack the pancreas and nearby tissues, causing episodes of pain. It often develops in people who are between the age of 30 and 40. The symptoms may mimic Pancreatic Cancer as well. CP is a global disease and studies depict that the incidences are increasing worldwide. CP occurs more often in men than in women and the condition often develops in people of age 30-40. However, there are cases where the onset of CP may occur even at a very young age. The chronic form of pancreatitis can be triggered by one acute attack that damages the pancreatic duct. The damaged duct causes the pancreas to become inflamed. Scar tissue develops and the pancreas is slowly destroyed.

According to available literature, chronic pancreatitis was first described in Kerala, a southern state in India and most of the cases have been reported from the southern states – Karnataka, Tamil Nadu and Andhra Pradesh. However, our patient data reports incidences of the diseases from many parts of the country.

UNSAID FACTS ABOUT CP
Before starting Ayurvedic treatment, case history was collected for all the patients enrolled during 1997-2015. Some of our observations revealed interesting facts that helped in better understanding of the disease. While a few clinical observations were similar to what we already know; a few added to our existing knowledge. For example, it is well-known that it is the alcoholics who develop chronic pancreatitis. However, our clinical observations suggest that most of the patients enrolled with us were vegetarians, non-alcoholic and non-tobacco users. Recently, Vitamin D3 deficiency in Indian population has been reported widely. In February 2015, we also screened patients for Vitamin D3 levels in their blood. Surprisingly, 90 percent of these patients showed deficiency. Though a few scientific studies* have attempted to study this aspect, the link between Vitamin D deficiency and chronic pancreatitis is still not well understood. Encouraging trend: a large number of patients want to get better and are opting for Ayurvedic treatment.
In early years, CP patients were being treated with Ayurvedic formulations for long years. Gradually, the duration of Ayurvedic treatment has been customized for one year. It is found effective in relieving symptoms of patients as well as in bringing significant change in their quality of life.

OUR OBSERVATIONS It was also interesting to see that soon after an event “Path of Innovation” that was organized at Dehradun in November 2014 to create awareness about the disease; there was a significant rise in the number of patients who aspired for Ayurvedic treatment for CP. Here is a glimpse of progress report – 100 cases from 1997 to 2015.
Available statistics indicate that the life expectancy of a majority of the patients suffering with Chronic Pancreatitis is about 9 years with moderate to severe physical and mental debility.
But after being put on herbo mineral Ayurvedic formulations, a significant improvement was seen in the quality and span of life among CP patients. It is a proud moment to share that our first patient who enrolled in 1997, has completed 18 years of symptoms free life. He is now leading a normal and a healthy life.

GENESIS GENESIS OF HERBO MINERAL AYURVEDIC FORMULATION FOR CP: AN INCIDENTAL DISCOVERY
Vaidya Chandra Prakash
Meerut-based Vaidya Chandra Prakash was a master of Rasa Shastra, who in his clinical practice used Ayurvedic medicines prepared at his residence under the able guidance of his Guru Maharaj ji. He used to put his intuitions into practice and developed many new formulations in his life time. In 1973, one of his herbo-mineral formulations brought miraculous recovery to a terminally ill patient of pancreatic cancer. Thereafter, the same formulation completely cured many patients suffering with Chronic Pancreatitis. Though these evidences were documented, it was only in 1997; it was done meticulously.
Following the data obtained from the clinical observations, details of those 45 patients who have completed Ayruvedic treatment, is presented here.

It was found that all these patients were diagnosed using modern diagnostic tools. 17 patients were diagnosed through MRCP and 12 through ERCP. Given that, CP is difficult to diagnose, MRCP and ERCP are better diagnostic tests over CT scan and USG.

It was observed that 12 out of 45 patients opted for Ayurvedic treatment after two attacks, including the ones who suffered one, three or multiple attacks. It is indicative that many of these patients resorted to Ayurvedic treatment.

TRADITION COMBINES WITH S&T Following the event ‘Path of Innovation’ that was organized on 23 November 2014, a project on development of herbo mineral formulation for CP was launched on 14 January 2015 under the aegis of Uttarakhand State Council for Science and Technology (UCOST), Govt. of Uttarakhand.
Hon’ble Cabinet Minister Shri Dinesh Agarwal, eminent scientist Padma Shri Dr AN Purohit, Chief Justice (High Court) KB Shahi, Shri Deepak Gairola, Secretary S&T, Shri Surendra Kumar (Media Incharge, CM of Uttarakhand), Dr Rajendra Dobhal (DG, UCOST) and many others were present on the occasion.
It is for the first time that a state government stepped forward to support drug development.

Shri Deepak Kumar Gairola, Secretary, Department of Science & Technology, Government of Uttarakhand made a visit to our Center in June 2015. He interacted with patients of chronic pancreatitis and took an overview of the ongoing state sponsored Drug Development Programme for Chronic pancreatitis at VCPC Research Foundation in Dehradun.
The 100th enrolled patient of chronic pancreatitis along with his father narrates their case and satisfactory experience with our treatment.
