RESUMO Os terceiros molares podem apresentar-se total e /ou parcialmente retidos, podendo ser mucosos, submucosos, ou completamente retidos no interior dos ossos maxilares maxilares ou na mandíbula. mandíbula. A técnica cirúrgica inclui um tipo de incisão, desempenhando um papel fundamental na cicatrização das feridas, que apresentam apresentam uma série de incisões descritas ao longo do tempo, por diferentes pesquisadores e autores, em uma tentativa de minimizar o seu impacto, empregar julgamento profissional de cada um, de acordo com suas necessidades e conveniências.

INTRODUÇÃO A cirurgia dos terceiros molares retidos total ou parcialmente é um dos procedimentos mais comuns na cirurgia bucal, entretanto requerem uma especialização, muita experiência e bom senso do profissional (FARISH; BOULOUX, 2000; OFFENBACHER; LIEFF; BOGGESS et al ., 2001; SUSARLA; LAESE; MAGALNICK, 2003; WAGNER; OTTEN; SCHOEN et al, 2005; ADEYEMO, 2006; GRAZIANI; D’AIUTO; ARDUINO et al ., 2006; GUILL GUILLAUME; AUME; L ACOSTE; ACOSTE; GABORIT et al ., 2006; KUNKEL; DOZ; MORBACH et al ., 2006; BLONDEAU; DANIEL, 2007; BOULOUX; STEED; PERCIACCANTE, 2007; KIRMEIER; TRUSCHNEGG; PAYER et al ., 2007; POZOS; MARTÍNEZ; AGUIRRE et al ., 2007; SUSARLA; DODSON, 2007; ADEYEMO; J AMES; OGUNLEWE et al ., 2008; MARZOLA, 2008; ABU-EL NAAJ; BRAUN; LEISER et al ., 2010; AMARILLAS; TORANZO; MARTÍNEZ et al ., 2010; KIM; CHOI; ENGELKE et al ., 2011 e LÜBBERS; MATTHEWS; DAMERAU et al ., 2011). Podem ainda acontecer nas caries não tratáveis e, na prevenção das lesões cariosas (MCARDLE; RENTON, 2000; SHUGARS; ELTER; JACKS et al ., 2005; AHMAD; GELESKO; SHUGARS et al ., 2008; MARZOLA, 2008; CHANG; SHIN; KUM et al ., 2009; OZEÇ; HERGÜNER SISO; TAŞDEMIR et al ., 2009; FISHER; MOSS; OFFENBACHER et al ., 2010 e GARAAS; MOSS; FISHER et al ., 2011), ou nas reabsorções radiculares dos dentes adjacentes (NITZAN; KEREN; MARMARY, 1981; YAMAOKA; FURUSAWA; IKEDA et al ., 1999; MARZOLA, 2008 e KAUSHIK; GUPTA, 2010) , ou ainda nas considerações ortodônticas com ou sem o apinhamento dos incisivos inferiores (HARRADINE; PEARSON; TOTH, 1998; AL-BALKHI, 2004; NIEDZIELSKA, 2005; BERNABÉ; FLORES, 2006; SIDLAUSKAS; TRAK, INIENE, 2006; BAGHERI; ALI KHAN, 2007 e MARZOLA, 2008) . Ainda, nas soluções de problemas protéticos (NANCE; WHITE; OFFENBACHER et al ., 2006 e MARZOLA, 2008) , ou na prevenção e / ou associação com as fraturas mandibulares (MARQUEZ; STELLA, 1998; LIBERSA; ROZE; CACHART et al ., 2002; WAGNER; OTTEN; SCHOEN et al ., 2005; MARZOLA, 2008; AL-BELASY; TOZOGLU; ERTAS, 2009; CHRCANOVIC; CUSTÓDIO, 2010 e GRAU; GARGALLO; ALMENDROS et al. , 2011) ou ainda, como uma solução profilática (ANDERSON, 1998; YAMAOKA; FURUSAWA; IKEDA et al., 1999; ADEYEMO, 2006; ALMENDROS; ALAEJ OS;QUINTEROS et al ., 2008; JASINEVICIUS; PYLE; KOHRS et al ., 2008; MARZOLA, 2008 e STATHOPOULOS; MEZITIS; KAPPATOS et al ., 2011). 2008; KHADER; AL-SHISHANI; OBEIDAT et al ., 2009; MADRID; BOUFERRACHE; POP et al ., 2009 e MATEVOSYAN, 2011) , ou ainda, nos cistos e tumores (AL-KHATEEB; BATAINEH, 2006; KUNKEL; DOZ; MORBACH et al ., 2006; BLUM; FRONT;PELEG, 2007; DOĞAN; ORHAN; GÜNAYDIN et al ., 2007; MARZOLA, 2008 e STATHOPOULOS; MEZITIS; KAPPATOS et al ., 2011). Suas indicações podem ser ainda quando interferem nas cirurgias ortognáticas ou nas reconstruções mandibulares (MEHRA; CASTRO; FREITAS et al ., 2001 e MARZOLA, 2008) , nos dentes para uso em determinados tipos de transplantes de germes dentais ou não (MARZOLA, 2008 e MARZOLA; PAGLIOSA, 2011), ou ainda quando estes dentes estão envolvidos no campo de recessão de um tumor (FRÄNZEL; GERLACH; HEIN et al ., 2006; MARCIANI, 2007 e MARZOLA, 2008) , ou quando se apresentam como dentes antagonistas que não desempenham ou desempenharão qualquer papel na oclusão (ZADIK; LEVIN, 2007; JASINEVICIUS; PYLE; KOHRS et al ., 2008 e MARZOLA, 2008). A controvérsia no tratamento dos terceiros molares é guiada por duas correntes filosóficas representadas pelos intervencionistas profissionais que são a favor da remoção profilática dos terceiros molares retidos total e /ou parcialmente livres de patologias assintomáticas, sustentada nas indicações para serem evitadas uma série de complicações, não havendo evidência científica, contudo, para afirmar ou negar que todos favorecem a saúde do paciente. Também, a escola oposicionista que combate esta política de tratamento (GUIDANCE ON THE EXTRACTION OF WISDOM TEETH, (NICE) 2000; METTES; NIENHUIJS; VAN DER SANDEN et al ., 2005; AL-KHATEEB; BATAINEH, 2006; FRIEDMAN, 2007; MARZOLA, 2008; PROPHYLACTIC REMOVAL OF WISDOM TEETH - A REVIEW OF THE CLINICAL BENEFIT AND GUIDELINES. CANADIAN AGENCY FOR DRYGS AND TECHNOLOGIES IN HEALTH. HEALT TECHNOLOGIY ASSESSMENT HTA. AGOST 2010) . Entre as complicações mais comuns encontram-se a ansiedade pré e pós-cirúrgica (GARIP; ABALI; GOKER et al ., 2004; JONGH; OLFF; HOOLWERFF et al ., 2008 e MARZOLA, 2008) , além da dor, o trismo, a infecção e, o alvéolo seco (ALMENDROS; BERINI; GAY, 2006; GBOTOLORUN; OLOJEDE; AROTIBA., et al ., 2007; AKADIRI; OKOJE; AROTIBA; 2008; MARZOL A, 2008 e CLAUSER; BARONE; BRICCOLI et al ., 2009). Como medida para serem evitados estes riscos e complicações, foi proposta que a técnica cirúrgica e, a experiência do cirurgião são essenciais para uma boa gestão de qualquer tipo de tratamento (BOULOUX; STEED; PERCIACCANTE, 2007; MARZOLA, 2008 e KAUSHIK; GUPTA, 2010). A importância do design da incisão deve ser sempre destacado na técnica cirúrgica dos terceiros molares retidos, pois vai depender dele a perfeita cicatrização da mucosa e, principalmente da neoformação óssea. Justifica-se este trabalho para todos poderem sentir que este detalhe é fundamental para o sucesso da intervenção, aspecto este que sempre é muito deixado de lado pela BLANCO BALLESTEROS, G. maioria dos profissionais, ou por não saberem mesmo ou ainda por se deixarem levar pela completa falta de conhecimento do assunto.
REVISTA DA LITERATURA Dentro da técnica cirúrgica sempre estará incluido o design da incisão, a ostectomia, a odonto-secção, a colocação ou não de dreno, a exposição intencional do alvéolo e, a técnica da sutura, que são os tempos essenciais a serem seguidos corretamente (MARZOLA, 1975, 1988 e 2008 e ERDOGAN; TATLI; ÜSTÜN et al., 2011). Diversos autores têm proposto diferentes tipos de design de incisões (MEAD, 1930; COGESWELL, 1933; AVELLANAL, 1946; WARD, 1956; MAUREL, 1959; KRUGER, 1959; RIES CENTENO, 1960; BERWICK, 1966; HOWE, 1971; SZMYD, 1971; MARZOLA, 1975; LOTTER, 1984; MARZOLA, 1988; BERZAGHI; CURSIO, 1989; STEVÃO; AZOUBEL, 1998; NAGESHWAR, 2002; SUAROLEZ; GUTWALD; REICHMAN et al., 2003; SILVA; CABRAL; FELIPE et al., 2006 e MARZOLA, 2008) (Figs. 1 a 27) e, cada uma delas tem sido utilizada para assegurar um acesso adequado, diminuindo os efeitos colaterais do processo, como a dor, o trismo, edema, alveolite e infecção e, também, reduzindo as complicações, tendo sido o objectivo das declarações de muitos estudos sobre estas incisões.

Outra grande preocupação são os defeitos periodontais podendo surgir na superfície distal ao segundo molar após a remoção dos terceiros molares retidos, sendo relatados em publicações sobre a frequência de doenças periodontais, relacionadas à idade, inclinação do molar, visíveis na placa das superfícies distal ao segundo molar (MARZOLA, 2008 e CETINKAYA; SUMER; TUTKUN et al ., 2009). Problemas periodontais, produzem resultados contraditórios de acordo comos estudos relatados e, enquanto alguns mostram uma diminuição na altura do osso distal ao segundo molar, após a extração do molar retido, outros estudos exibem que a altura é aumentada após a cirurgia e, podendo ser afetada pelo simples fato de levantar uma base na osteotomía (ROSA; CARNEIRO; LAVRADOR et al ., 2002 e MARZOLA, 2008) . A incisão não deve ser realizada em defeitos ósseos, ou cortar o músculo ou tendão e, estas incisões não devem ser muito extensas, pois poderiam influenciar as consequências infelizes da extração de molares (NAGESHWAR, 2002). O projeto utilizado durante a cirurgia para a remoção de terceiros molares retidos evita complicações relacionadas às condições periodontais ao segundo molar (KARACA; ŞIMŞEK; UĞAR et al ., 2007). Autores acreditam que este projeto tem uma influência primaria de cura, impedindo a deiscência da ferida e, sendo avaliada a técnica de sutura para alcançar este fechamento (SUAREZ; GUTWALD; REICHMAN et al., 2003). Acreditam ainda outros autores que o fechamento primário evita a drenagem da tomada, piorando a inflamação e a dor pós-operatória, optando por colocar drenos, com a obtenção de um doloroso e traumatizante pós-operatório (SANCHIS; HERNÁNDEZ; PEÑARROCHA, 2008). O invólucro apresenta uma incidência de deiscência da ferida, podendo induzir uma alveolite e o doloroso período pós-operatório, com incômodo, os danos no nível de fixação de fibras periodontais nível cervical e, quando realizada a incisão, o retalho triangular tem uma taxa de edema maior e, com melhor visibilidade operatoria (KIRK; LISTON; TONG et al ., 2007). Comparando dois tipos de retalhos em um estudo, o retalho em envelope apresentou maior inflamação e a aba em baioneta (triângulo) utilizado para comparar os efeitos da dor, edema, trismo, ferida deiscência após remoção de terceiro molar retido, o grau de inflamação por vários autores é influenciada pelo grau de irrompimento (parcial ou total) e, angulação, para eles não houve diferença, havendo muito baixa incidência de inflamação em comparação com molares na vertical e na posição medial, quando o grau de inflamação não estava relacionado com a duração do cirugía (SANDHU; SANDHU; KAUR, 2010). O tipo de incisão não tem efeito sobre a saúde periodontal distal ao segundo molar, podendo influenciar no primário da cura da ferida e, um grau de osteíte alveolar (ARTA; KHEYRADIN; MESGARZADEH et al ., 2011). Foi realizada uma comparação de dois tipos de retalhos (AVELLANAL, 1946; MARZOLA, 1996 e MARTINS; LEMES; ZARDO et al., 2007), não sendo encontradas diferenças significativas relacionadas com a cicatrização periodontal ao segundo molar a serem analisadas e consideradas outros fatores, como, idade do paciente, a superfície de contato entre 2 e 3 molar, tamanho do folículo pericoronário, a presença de inflamação periodontal pré-existente, a taxa de retenção, a quantidade de osso removido e, de gengiva inserida na distal ao segundo molar, a proximidade do ramo da maxila e do tipo de sutura e, a habilidade o profissional. Comparando dois tipos de retalho (envelope e triangulares), a primeira tem um menor grau de inflamação do que o segundo, não havendo nenhuma diferença significativa no grau de trismo entre as duas, tempo operatório, o número de analgésicos tomadas, recomendando a redução da aba do envelope por apresentar um maior índice de inflamação (ERDOGAN; TATLI; ÜSTÜN et al., 2011). Outros dois tipos de retalho foram comparados (Szmyd e triangular ou para marginal) não encontrando diferença na cura e, recomendando a remoção cirúrgica com trauma mínimo e, apresentando um retalho de gengiva intacta Szmyd distal ao segundo molar cura e periodontal em comparação outra aba após a extração de molares impactados completamente horizontais ou mesioangular (KIRTILOĞLU; BULUT; SÜMER et al ., 2007). Outro estudo concluiu que a deiscência do retalho tipo do envelope ocorre e isto afecta a cura primária, produzindo um período pósoperatório mais longo, doloroso e desconfortável para o paciente ( JAKSE; BANKAOGLU; WIMMER et al ., 2002). O fechamento secundário após a remoção de terceiro molar retido, fornecemdo resultados considerando as variáveis de dor e inflamação, reduzindo o desconforto pós-operatório e o fechamento primário poderia ser BLANCO BALLESTEROS, G. mais doloroso, causando osteíte alveolar (PASQUALINI; COCERO; CASTELLA et al ., 2005). O desenho da incisão preconizada numa adesão às técnicas de Avellanal e uma modificação poderiam trazer mudanças consideráveis na cicatrização, com resultados positivos com o sucesso da manobra (MARZOLA, 1975, 1988 e 2008). A incisão triangular posteriormente ao segundo molar inferior facilitaria a visão do local e, também, o acesso à região óssea, além de um local melhor para a incisão obliqua lateral que serviria como uma drenagem natural do local. Esta incisão oblíqua seria iniciada no ângulo disto-vestibular do segundo molar e desceria até o fundo de sulco (Figs. 25 e 26). Seria suturada apenas no ângulo, juntamente com a parte posterior e, não seria suturada até o fundo de sulco, permanecendo aberta, funcionando como um dreno permanente nos primeiros dias do pós operatório (Fig. 27). Resultados excelentes no pós-operatório foram verificados nestes mais de cinquenta anos de proservação (MARZOLA, 2008).
DISCUSSÃO Os aspectos técnicos científicos do design dos retalhos para a cirurgia dos terceiros molares são sempre discutidos pela grande maioria dos autores que os desenharam, sempre com um objetivo primordial de facilitar a cicatrização e impedir a dor e o grande edema no pós-operatório (MARZOLA, 1975, 1988 e 2008). Assim, todos eles preconizam suas incisões sempre com a finalidade de um aprimoramento e um sucesso na técnica cirúrgica (NAGESHWAR, 2002; INFANTE, 2006; KARACA; ŞIMŞEK; UĞAR et al ., 2007; KIRK; LISTON; TONG et al ., 2007; MARZOLA, 2008; SANCHIS; HERNÁNDEZ; PEÑARROCHA, 2008; LAISSLE; APARICIO; URIBE et al ., 2009; ARTA; KHEYRADIN; MESGARZADEH et al ., 2011 e ERDOGAN; TATLI; ÜSTÜN et al., 2011). As incisões não têm alterações periodontais distalmente ao segundo molar 6 meses após a extração, de modo que o projeto do retalho é uma questão de preferência individual do cirurgião e, também, do tipo de treinamento recebido (NAGESHWAR, 2002; INFANTE, 2006; KARACA; ŞIMŞEK; UĞAR et al ., 2007; KIRK ; LISTON; TONG et al ., 2007; MARZOLA, 2008; SANCHIS; HERNÁNDEZ; PEÑARROCHA, 2008; LAISSLE; APARICIO; URIBE et al ., 2009; ARTA; KHEYRADIN; MESGARZADEH et al ., 2011 e ERDOGAN; TATLI; ÜSTÜN et al., 2011) e, muitas técnicas exigem treinamento prévio para um manuseio correto (NAGESHWAR, 2002). Se houver algum problema com os tecidos moles em torno dos segundos molares, não é aparentemente um resultado da cirurgia ou da técnica, mas é o resultado, talvez, de outro processo. Outros estudos comparativos ainda são necessários para determinar a melhor técnica (ROSA; CARNEIRO; LAVRADOR et al ., 2002, SUAREZ; GUTWALD; REICHMAN et al., 2003 e MARZOLA, 2008). profundidade não está relacionada com o tipo de fechamento usado sendo o resultado de uma técnica mais conservadora (JAKSE; BANKAOGLU; WIMMER et al ., 2002; NAGESHWAR, 2002 ; ROSA; CARNEIRO; LAVRADOR et al ., 2002; PASQUALINI; COCERO; CASTELLA et al .; 2005; INFANTE, 2006; KARACA; ŞIMŞEK; UĞAR et al ., 2007; KIRTILOĞLU; BULUT; SÜMER et al ., 2007; MARTINS; LEMES; ZARDO et al., 2007; MARZOLA, 2008; CETINKAYA; SUMER; TUTKUN et al ., 2009; SANDHU; SANDHU; KAUR, 2010 e ARTA; KHEYRADIN; MESGARZADEH et al ., 2011). Os poucos estudos que realizam uma comparação entre diferentes técnicas de incisão não encontraram nenhuma diferença significativa sobre os efeitos adversos da cirurgia de terceiro molar e mais, a perda periodontal anexo, índice de placa, nível ósseo distal ao segundo molar, margem gengival, profundidade de sondagem, edema, dor, trismo, abertura da boca e, assim, não há diferenças significativas no que demonstrem que uma incisão deva ser utilizada preferentemente (JAKSE; BANKAOGLU; WIMMER et al ., 2002; NAGESHWAR, 2002 ; ROSA; CARNEIRO; LAVRADOR et al ., 2002; PASQUALINI; COCERO ; CASTELLA et al ., 2005; INFANTE, 2006; KARACA; ŞIMŞEK; UĞAR et al ., 2007; KIRTILOĞLU; BULUT; SÜMER et al ., 2007; MARTINS; LEMES; ZARDO et al., 2007; MARZOLA, 2008; CETINKAYA; SUMER; TUTKUN et al ., 2009; SANDHU; SANDHU; KAUR, 2010 e ARTA; KHEYRADIN; MESGARZADEH et al ., 2011) e, por outro, melhorar a qualidade de vida dos pacientes.
CONCLUSÕES Há uma variedade de propostas para abordar incisões de terceiros molares retidos, mas as conclusões não são claras e os empregos tendem a ser controversos e, muitos estudos carecem de um pequeno número de casos e, variáveis sob investigação podendo ser exclusivas, além de muita conversa sobre a experiência do cirurgião para evitar complicações e, tempos operatórios. No entanto todos os procedimentos sempre relatam complicações, trismo, dor, edema, dificuldade em abrir a boca e, outros não levam em conta fatores como de vida dos pacientes, afetou significativamente. Pode-se concluir que: 1. As incisões não têm alterações periodontais distalmente ao segundo molar 6 meses após a extração, de modo que o projeto do retalho é uma questão de preferência individual do cirurgião e, o tipo de treinamento recibido. 2.
