Workers must not be permitted to use or operate any lifting equipment unless they are instructed, trained and qualified by a competent person in the use and operation of the equipment. Documentation of contractor qualified crane operator qualifications must be provided upon request as per the contract. Are Gin Pole Trucks required to have a weight indicator? o If the weight of the lift has been confirmed by a shipping manifest or if the company has weighed each component and has documentation of this, NO a weight indicator is not required. o If the weight is not known a dynamometer can be used in place of a weight indicator. Are Gin Pole trucks required to have an owner’s manual? o When a Gin Pole truck has been certified they will have an owner’s manual specific for that Gin Pole Truck. The owner’s manual is required to be in the vehicle Are Gin Pole trucks required to have a Load Chart? o Yes – the load chart will be included in the owner’s manual. Can we make the entire lift with a dynamometer hooked up? o 5.7.1.1 Dynamometers shall not be used under dynamic conditions. Moving a load while hooked up to the Gin Pole truck is a dynamic lift. What is a dynamic lift? Dynamic means energetic, capable of action and/or change, or forceful, while static means stationary o or fixed. How often do these dynamometers need to be calibrated? o As per manufacturer, usually annually. Do the operators of a Gin Pole/Winch trucks have to be qualified? o YES – 5.1 Safety Precautions – Workers must not be permitted to use or operate any lifting equipment unless they are instructed, trained and qualified by a competent person in the use and operation of the equipment. Do the operators of a Gin Pole/Winch trucks need to carry a card showing that they are qualified to operate them? o Documentation that they have been trained on the lifting equipment is required. They may chose to carry a card or have a roster of the training available in the vehicle Do the operators of a Gin Pole/Winch trucks need to be a qualified rigger? o If they are hooking up the load – YES and they are required to have this card on site. Are Digger Derricks covered under the new OSHA regulation? o NO - 1926.1400(c) Exclusions (4) Digger derricks when used for augering holes for poles carrying electric and telecommunication lines, placing and removing the poles, and for handling associated materials to be installed on or removed from the poles. Digger derricks used in such pole work must comply with either 29 CFR 1910.269 (electric lines) or 29 CFR 1910.268 (telecommunication lines). As noted above, the operator needs documentation that they have been trained to operate the equipment and the Qualified Rigger rules still apply.
Do spreader bars need certification tags on them? Some Spreader Bars may not have a certification tag on them. If there is not a tag, there will o be a serial number either stamped or welded on the bar. The serial number will also be on the certification paperwork that was provided by the manufacturer. The certification paperwork shall be on file at the company owner’s office. If the spreader bars do not have the serial number on them, they are not considered Certified. To get the Certified, a Registered Professional Engineer or Structural Engineer will need to certify them. When do Auto Cranes need to be certified? The shall be inspected at least Annually by a c rane inspector o

The data and information to be provided on the Assembly Completion Date sticker should include, but not be limited to the following: a) Assembly Completion Date of Winch Truck b) Truck Make: c) Truck Model: d) Bed serial Number: e) VIN Number: f) Rated working load

Drilling rigs use a personnel lifting device. Are these considered lifting equipment? o

“YES” According to the L&R Standard if we are using Hoists to lift personnel they will need to follow 5.10 Scope This Lifting and Rigging Safe W ork Practice (SWP) Standard covers work performed by Chevron employees and their delegates and contractors within Chevron Global Upstream Exploration and Production (GU) operational control. This standard applies to mechanical lifting activities where lifting methods and rigging shall meet these minimum requirements.

Material and Personn el Hoist s
This section provides guidance for protecting workers using personnel, material hoists and winches. Only dedicated hoists and winches are used for personnel lifting, and those hoists and winches must not be used for any other purpose. 5.10.1.1
Inspections
All Personnel hoists must be: Visually inspected by a qualified inspector before daily use. Inspected and the documentation written at 30-day intervals. Tested in accordance with manufacturer specifications, local regulatory authority requirements, and with applicable ANSI requirements. Hoists dedicated for personnel lifts must be inspected and tested according to local regulations, but not greater than at three month intervals. Records of inspections and tests must be maintained and filed at the work site. •
•
For Auto Cranes rated at 3200 pounds or less, where can we get crane operator training for them? o Training for crane operators operating an auto-crane rated at 3200# or less can be trained by a competent person. Documentation of the Crane Operators training shall be kept on file at the local FMT – 5.8.8 in the L&R Standard. Does the tag line need to be a specific size or length? o NO- The standard does not specify the size or length of the tag li ne. 5.6.12 Tag Lines Pre-use Inspection o Tag lines should be attached to the load (not to the sling).

MCBU Lifting & Rigging Questions and Answers Updated July 12th, 2012 o For large loads, two tag lines are recommended with one being placed at each end of the load. o When using a tag line to direct a load into place, be aware that the load can swing into other objects when there is too much force applied in the wrong direction at the wrong time. Pull easily until the load turns and then direct it into place by using only enough force to get it there. Special lifting configurations may require longer tag lines. o In cases where a tag line may not be considered a safe option, the situation shall be discussed and included on a JSA and in a pre job safety meeting. Tag lines shall be free of any knots, splices, or loops, except for attaching to load. •

Do we need to do the 5 minute test (trial lift) at the start of each day with a Man-riding work basket? There isn’t anything in the standard about doing the trial lift at the beginning of the day or o shift, below is from the standard. At a minimum, the operator shall inspect the crane and outriggers for any settling, the rigger is to inspect the basket. 5.6.22 - If repair or replacement is deemed necessary, a new trial lift will be necessary before using the man riding work platform once repair(s)/replacement(s) have been implemented. Each time there is a need t o move to a different worksite, or the man riding work basket had been detached from the crane rigging , or the scope of work basket movement has been changed, a trial lift shall be completed prior to the manned lift. If we have a man basket that has been designed by the Registered Professional Engineer, do they need the weights for the pre-use c heck? YES – If personnel are going to be lifted in the Man-basket, they have to follow 5.6.22 in the L&R Standard which states “Qualified Crane Operators must conduct a trial lift to 125 percent of the Safe Working Load (SWL) of the man basket immediately prior to any personnel entering the basket. The Crane Operator shall move the unmanned work basket through all anticipated crane movements and basket path operations during the trial lift. The trial lift shall keep the work basket suspended for at least f ive minutes before re-landing the basket to grade.” What information is required on the certification tags for Contractor owned slings? •

5.11.4.1 All wire rope slings and synthetic slings owned by third-party contractors will contain identification tags consistent with the required information outlined below:

5.11.2.2 - Can I (plant operator) do the annual sling inspection on Chevron owned slings? o At the minimum you must have completed Qualified Rigger training. Until you get the required training this will have to be completed by your third party i nspector. How often does the “running wire rope” need to be changed out? 5.11.1.3 - The service life of all running wire rope will be verified during routine monthly, quarterly, annual, or heavy lift inspections. All running wire rope found to have been in service for 5 years or more (from installation date) at time of inspection will be replaced in a timely manner. Does a tag-line have to be rope or can we use a synthetic strap or chain? o
3.0 Terms and Definitions - A Tag line is - A length of rope attached to the load that is used by the qualified riggers to aid in the control of the load.

MCBU Lifting & Rigging Questions and Answers Updated July 12th, 2012 Do all Gin Pole Trucks need to be certified? o All Gin pole trucks under Chevron’s Operational control are required to be certified. Does the Qualified Rigger need to carry a card? Yes – 5.7.1 in the MCA L&R Standard - Contract Qualified Riggers are required to o provide proof of qualification at the work location. Are the belts guards on the pumping units certified for lifting? The belt guards on the pumping units are considered part of the pumping unit which is an o engineered piece of equipment from the manufacturer. The qualified rigger shall inspect the lifting eye on the belt guard prior to lifting it to ensure it is safe to lift. Can we put tape over the certification tag on the synthetic straps to protect them? Yes – It is fine to put tape over the tag. The tape will have to be removed prior to each o use of the sling to verify the slings lifting capacity and all other information. Do winch trucks need to be certified? o Yes, by definition from the IADC they are used for lifting. Chevron’s lifting and rigging standard and the OSHA construction regulation states that lifting equipm ent shall be certified. If I am doing work where overhead power lines may pose a threat, such as installing a transformer, does a Critical Lift Plan (CLP) need to be filled out? o NO – A critical lift plan is for a blind, complex, complicated or heavy lift. A PTW is required where overhead power lines pose a threat. If we have a homemade lifting device to remove and set an electrical motor which is in a tight spot, can we still use it? o 2.0 NO – Any Lifting devices shall be certified, which means they will be stamped and the certification paperwork will be on file. Is one-Call required when outriggers are set for mobile crane operations? o 5.4 Verification of buried electrical lines is required. Does the hand signal chart need to be posted on the back of a gin pole truck? o NO – 5.5.11 it needs to be conspicuously posted in the vicinit y of the hoisting operations Is the “color code” identifier for annual sling inspections an OSHA requirement? I could not find this in the OSHA regulations, although it is a Chevron requirement. o Where can we get a copy of the Critical Lift Plans? o All of the MSW forms are available on Chevron’s Forms management web-site. As a contractor you can ask a Chevron employee for any of these forms. How many times a year are the Synthetic straps Inspected? o All slings shall be visually inspected by a qualified rigger before each lift, and whenever they may have been damaged during a lift. o

Slings shall be inspected by a Qualified Rigger, in accordance with local regulations and requirements. Best practice for sling in spections is six month intervals, but shall be inspected at a minimum of twelve month intervals. This record will be maintained for the life of the sling.
Once the annual inspection on Chevron Owned slings has been completed, the MCA annual sling inspection form shall be completed and the sling shall be color coded with the color noted on the MCA Annual Sling Inspection color code diagram.

It is a requirement of each SBU to clearly identify and communicate the weather parameters for each work location and/or individual crane set-up. This table above is a guidance, and may be changed for individual lifts depending on the conditions, such as lift type, size, geometry of the load, location, visibility, temperature, etc. •
Slick line uses the hose to guide the lubricator instead of a tag line, is this ok? 3.0 Terms and Definitions (Tagline) NO – attach a tag line and use the rope to guide the load. When riding in a man-riding work basket where should I tie-off? 5.1 The individuals inside of the personnel basket need to be tied-off to the manufactured tie-off points in the basket. It is an option to tie-off to the cable above the load block of the crane. Are we required to carry cards for the crane operators? o 3.0 Yes – the crane operator is required to carry a card for the “Type and Capacity” of crane they are trained for. If we have hooks welded to a backhoe bucket and signed off by a Registered Professional Engineer, are these considered “Certified” o “NO” 5.8.1 - No modifications or alterations that affect the capacity or safe operation of the equipment can be made without written approval from the manufacturer. •

Does the Swing radius of the crane need to be barricaded? YES – 5.8.3 - Accessible areas within the swing radius of the rotating superstructure counterweight of a crane must be barricaded to prevent workers from being struck or crushed by the counterweight. •

What is meant by a “color code” for slings? 5.12.1..1
Identif icati on Codes
A color and/or shape code is a means to easily identify a component or item of rigging gear to show the current inspection status. Each company shall use a minimum of three colors and/or shapes, which shall be conspicuously displayed at the workplaces. The color and/or shape code identification should not be applied directly to the load-bearing part of a synthetic sling; it should be placed on the identification tag in a way that does not obscure an y critical data. The color and/or shape code m ust be changed every twelve months, following its inspection. Details of the c urrent color and/or shape code must be placed conspicuously around the work place.

What is the definition of a Competent Person? OSHA defines a competent person as one who is capable of identifying existing and predictable hazards in the surrounding or working conditions, which are unsanitary hazardous, or dangerous to employees and who has authorization to take prompt corrective measures to eliminate them. By way of training a nd/or experience, a competent person is knowledgeable of applicable standards, is capable of identifying workplace hazards relating to the specific operation, and has the authority to correct them. Some standards add additional specific requirements which must be met by the competent person. What do the different levels mean for a Qualified Rigger? CERTIFIED RIGGER LEVEL I NCCCO identified the following job duties for Rigger o Level I certification. A Certified Rigger Level I can perform simple, repetitive rigging tasks when the load weight, center of gravity, the rigging, and rigging configuration are provided or known by the rigger through experience or on-the-job training prior to the rigging activities. Specifically, Level I Riggers shoul d be able to demonstrate or have knowledge of how to: o

Inspect rigging before use Identify and attach rigging with basic knowledge of hitch configurations, capacities, and basic knots Recognize associated hazards Signal operations Use various types of rigging equipment and basic hitches and their applications CERTIFIED RIGGER LEVEL II In addition to knowing and demonstrating Rigger Level I knowledge and skills, a Certified Rigger Level I I can select rigging components and procedures based on rigging capacity. A Level II Rigger can perform the following rigging tasks unsupervised: Estimate load weight and center of gravity Identify lift points Determine and select rigging based on loading Perform pre-use inspection of rigging and lift points • •

How often do crane operator’s need to be re-certified? I heard every year. 1926.1427 According to the new OSHA regulation crane operator recertification is required every 5 years. •

If a mechanic does other work than “maintenance” work, will this be considered Construction work? This will be up to the individual – here is OSHA’s definition of Const. vs Maint. OSHA's regulations define "construction work" as "construction, alteration, and/or repair, including painting and decorating." Construction work is not limited to new construction. It includes the repair of existing facilities. The replacement of structures and their components is also considered construction work. "Maintenance activities" can be defined as making or keeping a structure, fixture or foundation (substrates) in proper condition in a routine, scheduled, or anticipated fashion. This definition implies "keeping equipment working in its existing state, i.e., preventing its failure or decline." However, this definition, (taken from the directive on confined spaces) is not dispositive; and, consequently, determinations of whether a contractor is engaged in maintenance operations rather than construction activities must be made on a case-by-case basis, taking into account all information available at a particular site. Do I still need qualified rigger training if the crane is rated at less than 2000# maximum lifting capacity? YES – the lifting capacity of the crane does not matter, if they are involved in the lift the minimum requirements are to be a qualified rigger. •

Do I have to be qualified to operate an overhead hoist? YES – OSHA 1926.1427 & 1430 Operators shall be trained •

Is a “winch truck” certification required by OSHA? I could not find this in the OSHA reg for Construction. It is required to be certified through the Chevron L&R Standard and according to the IADC guideline to gin pole truck operations.. Can I operate a crane if I do not have the accredited training for a certified crane operator? 1910.1427 Yes – If you already have been certified to operate a crane you can continue th to do so until November 8 , 2014. OSHA has given a 4 year window for all crane operators to be in compliance with the new accredited training for certified crane operators. If an auto-crane has the maximum lifting capacity over 2000# and if it used for construction work, is the crane operator required to com plete the accredited training for certified crane operators? Yes When using a man lift baskets, do the cranes lifting capacities have to be reduced? YES 1926.550(g)(3)(i)(B) •

Updated July 12th, 2012 Load lines shall be capable of supporting, without failure, at least seven tim es the maximum intended load, except where rotation resistant rope is used, the lines shall be capable of supporting without failure, at least ten times the m aximum intended load. The required design factor is achieved by taking the current safety factor of 3.5 (required under 1926.550(b)(2) and applying the 50 per cent de-rating of the crane capacity which is required by 1926.550(g)(3)(i)(F). Is a winch truck considered L&R equipment? o Yes – the load is still being lifted so it does apply to this standard. This means the equipment shall be certified by the m anufacturer or a Registered Professional Engineer. Do slings have to be sent in and Proof tested (pull tested) annually? NO – see boxes below - From ASME B 30.9 o o Chain SLings

Is the L&R information we received going to be included in the General Orientation? I will visit with L&D on this. o Do the CA Auditors need to be Qualified Riggers? “NO” – The CA auditors are not actually inspecting the equipment, they are ensuring that o the L&R equipment has been inspected. If an individual goes through the Certified Operator Training, is the Qualified rigger training included in that? o “NO” – Certified Operator training touches on the Qualified rigger training, but does not qualify them as such. Qualified Rigger training is a different class. Are the belts guards on the pumping units certified for lifting?

The tape will have to be removed prior to each use of the sling to verify the slings lifting capacity and all other information Do the Overhead hoists need an inspection tag on the bridge, which can be 20’ in the air? o NO – the annual inspection sticker should be located around eye-level preferably close to an entrance of the building. How long is the operator training for the Auto-Cranes good for? This is up to the company that provides the training. o Do the LLS (Load Limit Switches) on the Auto-Cranes need to be tested annually and is Universal valve and crane testing them? According to the inspector from Universal Valve and Crane the LLS is not tested o annually. The LLS is designed to stop the load if the auto-crane is overloaded, but will only do so a few times and then it will not reset so the switch will need to be replaced. If the switch does not reset, the auto crane will not pick up anything until the switch is replaced. Need to follow the manufacturer recommendations from the owner’s manual. The Nabors Rig uses a “hobble” that has been designed by Crosby to remove and install the Horses head. Can we still use this? This lifting device has been engineered and certified so it can be used. o Is the water well pulling unit certified? Pulling units are certified from the manufacturer so there should not be any issues with o them, unless they have been locally modified. If there have been any modifications, this will have to be re-certified from the manufacturer. If the safety latch comes from the manufacturer with a pin in the latch does it need to be in place prior to use? YES Is the Accredited Crane operator training classes available in Spanish? I was unable to find any companies that provide this – The regulation states that the training can be provided in any language, but I was only able to find classes in English. When the winch trucks move frac tanks, they are securing the load with tie-down chain, does this chain need to be certified? No, as long as it is tie-down chain, it does not need to be certified, but it still needs to be inspected prior to use. Is it required to have the certification paperwork for chain slings? •

No – some companies provide it and will be provided by other companies if the customer requires it. It is a requirement to have a certification tag on it with the minimum information.
For Overhead Hoists, can the lifting capacity of the Hoist or hoists be higher than the lifting capacity of the bridge? o NO After equipment such as Gin Pole trucks are certified, how often do they need to be recertified? Annually Our company does monthly sling inspections and the documentation is in each vehicle – do we still need to do the annual color code or shape identifier? YES – it is a requirement that an annual sling inspection take place. This is for all slings, even one’s that are in storage or not being used. Once the item has been inspected, it needs to be identified by either a color code or shape identifier. •

If the operator has training & documentation of the trai ning for overhead hoists then they can use our hoists. Are we going to be able to use gin pole trucks after the end of this year? All lifting devices, including gin pole trucks need to be certified by year end 2011 or they cannot be used on Chevron’s location. •

Can we sign permits related to L&R? YES - If the individual has completed the MSW training to become a Permit Approver then they can sign permits. Do auto cranes need an annual inspection? YES all L&R equipment needs an annual inspection. Refer to the owner’s manual for other requirements. Can a single hooked chain sling be certified and pull tested? Yes these lifting chains can be proof-tested and tagged (certified) Are Matts or pads required on all mobile crane outriggers. Yes o Can an OSHA 501 instructor train the Qualified Rigger class? o They can train the Qualified Rigger class if they have been through the Train the Trainer class for Qualified Rigger/Signalperson training. Can I operate a crane if the Anti-2-block does not work? o “NO” the anti-two block needs to be operational. What is the definition of a “lift” to move or bring (something) upward from the ground or other support to a higher o position; hoist. (dictionary.com) What steps need to be taken if a crane is “shock loaded” If a crane boom or structure is shock loaded during operation, or there is any indication of shock loading, all structural components of the crane s hall be inspected to detect cracks and other damage. Nondestructive test equipment, such as magnetic particle or ultrasonic procedures, is recommended for this inspection. Does the owner’s manual for the crane or back hoe have to be in the vehicle? o YES – Owner’s manual shall be in the vehicle and the operator needs to have a signed document attached to the owner’s manual stating that they have read the owner’s manual. Does a tag line need to be certified? o No- but the tag line needs to follow the MCA L&R Standard which states that it needs to be hooked to the load not the slings, free of knots except for the attaching end, if used for electrical services, be of a non-conductive m aterial and not wrapped around the body at anytime. Can we cut a hole with a torch in a pit tank to hook up slings for removal. o NO – The design to remove these pit tanks needs to come from a Registered Professional Engineer. •

I visited with NM OSHA and the training in NM is the same as all other states, The o person training the class must have received a train the trainer class for a Qualified Rigger / Signalperson. These classes are availabl e through American Crane, The Crane Institute and the NCCER On the electric motors we are drilling and tapping a hole to put lifting eyes in them. This lifts the motors more evenly. Do we need to go through the manufacturer for certification? o YES – any modifications or alterations to the equipment cannot be made without written approval from the manufacturer. How long is the Qualification for the “mechanics Cranes” good for? This depends on the company providing the training. One company that I talked to said o theirs is good for 3 years another companies training is good for 2 years.. If we have someone in the field that is a Qualified Rigger / Signalman, can they train other personnel in the field? NO – At the minimum the person training the class must be a competent person or has o received a train the trainer class for a Qualified Rigger / Signalperson. T hese classes are available through American Crane, The Crane Institute and the NCCER. As a Permit Approver do I inspect the slings and other Lifting devices? NO- Inspecting the slings for the Pre-Use inspection i s considered part of the job and o needs to be completed by the Qualified Rigger. The Permit Approver needs to verify through the qualified rigger and the Pre-Use inspection that the slings have been inspected and are certified by having a certification tag on it. Can we use a common drawing for all Pad-eyes on tanks? If Lifting eyes are being installed on tanks or vessels in the field, they still need to be o designed and stamped by a Registered Professional Engineer (RPE). A common drawing may be used only if each tank has the same dimensions. Can a tie-down chain be used as a lifting chain and vice-a-versa? Lifting chain can only be used for lifting and shall be a grade 80 or higher chain. Lifting o chain shall have a certification tag attached. Although it is not recommended, lifting chain can be used as tie-down chain. Do boom truck operators that operate trucks owned by electrical companies need to have the accredited crane operator training? o NO – The operators do need to be trained by a competent person. Trucks that are certified for electrical service work are exempt from the new OSHA Standard, but do need to follow OSHA Regs for these trucks. Does this training make us a Qualified Rigger or Signalman? o NO – this training is onl y an overview. The one slide said that the Signalman should wear a high visibility vest; does this vest need to be FRC being it is an outer layer of clothing? o Yes if your company decides to use the vests they will need to be FRC and these vests are available in FRC material. This is a best practice suggestion for the vest; some companies have chosen to use a different colored Hard Hat or different colored gloves. The signalperson needs to be identified during the JSA and all others need to know who is to give signals. Would slick line and Cap String jobs be considered routine being the crews are out here doing these jobs all of the time? o This is the call from the OS, PA and WTL. If it is a job that is done on a regular basis it can be classified as a routine job. Although each FMT has the right to complete a Critical Lift Plan for any job. If a company has a manufactured lifting device to slide over the forks on a fork lift can this be used and does the L&R Standard need to be followed? •

Updated July 12th, 2012 o Yes - The manufactured lifting device shall be certified and designated for that particular forklift. The operator will need to be trained as per the forklift regulations. Any Lifting devices shall also be certified. Are the electrical power lines inside of the trays in the plant considered “Overhead Power Lines”? o YES – the lines are energized and a PTW is required when lifting where Overhead power lines may pose a threat. Is there any way that a sheet similar to the green sheet can be made up, just for L&R? o Critical Lift Plans are now two-sided and available on Forms Management There is confusion between routine and non-routine lifts. Why does the standard even mention routine lifts if every crane lift on land is considered a non-routine lift? The L&R standard is written for GUG, in the MCA the majority of crane lifts are considered o non-routine being they are not done very often. Non-routine lifts do require a PTW. Does certified L&R equipment require certification paperwork to be filed? Most certified lifting equipment will have a certification. If a certification was delivered with o the L&R equipment then the certification paperwork shall be fi led either in the Chevron or Contractors office (s) for the life of the equipment. All slings and devices designed for lifting from a Registered Professional engineer will have the certification paperwork. Rigging devices such and Shackles and Eye-bolts do not have paperwork, but do have information stamped on the devices. If there was an accident during a Lifting & Rigging job task and OSHA becomes involved, which Regulation will they follow, Maintenance or Construction? According to OSHA (WY & Fed OSHA)– It will depend on what activity you will actually be o doing, but they will lean towards the more stringent regulation which is the Construction Regulation. Is rigging up & down for oil & gas drilling and servicing considered Maintenance or Construction? This is the call of the supervisor of the job. o How do we know if the lifting eyes are certified? If the Lifting Eyes (pad-eyes) came on a piece of equipment from the manufacturer and the o container is certified then the lifting eyes are certified. If the Lifting eyes have been modified in any form other than by the manufacturer then the lifting eye is considered a home-made lifting device and cannot be used after year-end 2011, unless it has been recertified by the manufacturer or a Registered Professional Engineer. A pre-use inspection b y the Qualified Rigger shall be performed to look for any abnormalities. If someone inspects slings or hooks up the slings to a load, do they need to be trained as a Qualified Rigger? YES, anyone who hooks up slings or inspects them prior to use or for the annual inspection o are required to be a Qualified Rigger. Does the person guiding the load by using a tag line need to be a Qualified Rigger? YES, 3.0 terms and Definitions o In the future, will L&D provide Qualified Rigger and Signalman training for Chevron employees? o Paul will talk to L&D and provide an answer by the end of May - 2011 o The use of drawings provided from the plant or a Chevron location is considered verification. If synthetic straps are written on with a permanent marker, fabric dye or stencil ink, do they have to be destroyed? o NO – although it is not recommended, writing on them with a permanent marker, fabric dye or stencil ink will not reduce the lifting capacity or destroy the threading. We use a pick-up and lay-down tool for well work, the tool itself is certified, but all other lifting parts are not, do these other l ifting parts need to be certified? •
YES - 2.0 REQUIREMENTS - Lifting and rigging equipment must be engineered and certified for current use and in good working order as verified through pre-use inspections.
Note: The use of non-certified locally fabricated or modified lifting and rigging equipment is prohibited. •
Does a pulling unit sand line require a weight indicator? “NO” – the sand line is only used for swabbing. Does a pulling unit winch require a weight indicator? “NO” - each winch is rated by the manufacturer with a rated lifting capacity. Being it is a requirement to know the weight of the load prior to picking it up, i t is the operator’s responsibility to ensure that the winch is not over-rated. On a rig, is the winch certified for lifting personnel “NO” The winch cannot pickup personnel Does this apply to rigs? o Yes - Any L&R equipment needs to be certified, anyone involved in guiding the load or hooking it up needs to be a qualified rigger. If they are giving hand signals they need to be a qualified signalperson. Rig personnel still need to follow the L&R Standard. •
