3. SUMMARY The scope of this document is to provide guideline to the team performing Lifting Plan during site preparation for ….. The work will be included: Crane Pre-Lift Inspection Checklist. Operation or simulation test refer to available SOP. 4. HES PRECAUTION All steps in this procedure shall be performed in accordance with and under the constraints of Torishima Safe Work Practices (SWP) and applicable local safety rules and regulations. Warning: STRICT ADHERENCE TO THE DETAILS INDONESIA LABOR LAWS MUST BE MAINTAINED AT ALL TIMES DURING THE IMPLEMENTATION OF THIS PROCEDURE. THE SITE AND LOCAL SAFETY GUIDELINES AND PROCEDURES SHALL BE ADHERED TO AT ALL TIMES. IT IS ADVISABLE TO USE THE LOCAL NDC STANDARD OPERATION PROCEDURE OF OVER HEAD CRANE OF MAINTENANCE, TAGGING AND PADLOCKING OF EQUIPMENT, WITH ALL PERSONNEL MADE FAMILIAR WITH THE PROCEDURE. IT IS OF PARAMOUNT IMPORTANCE THAT ALL PERSONNEL IN THE VICINITY OF LIFTING & RIGGING AND WORKING AT HIGH. 5. PRE-REQUISITES: The following prerequisites shall be completed prior to performing Lifting Job using Crane at HCT Plant : Ensure to do Crane Pre-Lift Inspection Checklist before operate crane Lifting Plan should be done prior to lifting operation start and Lifting Plan The weight of the lifted load and its center of gravity must be known with reasonable accuracy early in the planning process Special consideration for ground support capacity for the crane Before perform lifting ensure that atmospheric test is doing. Lifting Equipment to be used should have a valid certificate and color code Use appropriate lifting equipment All crane crew should be certified personals: Crane operator should have a valid certificate as a MIGAS Crane Operator Lifting Supervisor, Rigger and Signalman should have a valid certificate as a Lifting Supervisor, Rigger and Signalman All crane crew should be get safety induction as SWP required 6. RESOURCES a) Personnel Qualification o Supervisor : 1 person o Crane Operator : 1 person o Rigger : 2 persons b) Tools and Materials Required o Lifting gears ( wire rope sling, shackle, etc ) o Plastic rope for tag line. 7. PROCEDURE Preparations: Persiapan : Prepare SRP, JSA, Permit, Check ATC and conduct tail gate meeting between Crane crew who will perform this job Siapkan SRP, JSA, Permit, Check atmosphere dan lakukan tail gate meeting bagi crew crane yang akan bekerja Make sure that all of PPE are available and used. Pastikan semua PPE yang diperlukan tersedia dan dipakai Pre – Lift Inspection Checklist: Pemeriksaan sebelum pengangkatan Check and cleaning of crane unit and ensure there is clear and no foreign material. Cek dan bersihkan crane dan pastikan tidak akan ada barang terjatuh bila crane dioperasikan Make sure the barricades have been installed properly in lifting operation area Pastikan barikade telah terpasang dengan baik di area pengangkatan Ensure that the pre-use inspections have been carried out on cranes that will be used and all finding have been resolved Pastikan bahwa pre use inspeksi telah dilakukan untuk crane yang akan digunakan dan semua finding telah diselesaikan Make sure had been disconected and free from obstacle Pastikan telah tidak terhubung dan bebas dari penghalang apapun For All Terrain crane Ensure that timber mat for crane’s jacks are installed Pastikan timber mat sudah terpasang pada semua kaki2 Ensure that the outrigger was full extention prior lifting operation ongoing Pastikan bahwa outrigger telah full extention sebelum lifting operation berlangsung Ensure that the ground support condition to avoid outrigge r collapsed to the ground. Check overall wire rope and drum condition, ensure that is in place and in pulley groove. Cek kondisi sling dan drum sling, pastikan sling berada di groove Check hook condition and ensure the safety latch in good condition and function Cek kondisi hook dan pastikan ada safety latch serta berfungsi Check hook block condition Cek kondisi hook block Check pulley of hook block condition Cek kondisi pulley pada hook Operate the crane swing smoothly. Jaga kebersihan crane dan area kerja Lifting the load: Ensure the operator and rigger are certified, proper lifting gears are checked and marked as color coding Pastikan operator dan rigger punya sertifikat, lifting gear sudah diperiksa dan diwarnai sesuai color coding Operator should really know the weight of starting package to be lifted Operator harus benar-benar mengetahui berat starting package yang akan diangkat Set cranes in safe operation, set the working radius for free swing movement, all outrigger jacks are fully extended for max load Set Cranes pada kondisi aman untuk dioperasikan, pastikan kaki2 terpasang dengan sempurna Arrange the crane’s position so that it is safe to lift the . .
