To improve the quality and intergrity of data collected in every inspection conducted at site. To verify compliance to regulatory requirements. requirements.

EFFECTIVE DATE AND BENEFITS OF NEW TAKING OVER FORMS Development Department has got the approval from EXCO in regard to Taking Over Forms. Effective 1st January the new Taking Over Forms will be in use for the project that kick off in January 2016. The benefits of a new inspection forms are as follows: to minimize the risks associated with regard to manipulation of data by human factor, to verify compliance to regulatory requirements and SOP, including good practices, to validate the integrity of the data generated, to request corrective actions from the inspected party whenever problems are noted, to take compliance actions when deemed necessary. •

1- QT3 (Material Test and Inspection) Competent Person fill up QT1/Sixth Schedule (standard application letter) to apply for material inspection. District notifies the date and time for the inspection using standard notification letter QT2. QT3 will be filled and signed off at site by designated personnel. Development Unit at District must witness the test together with CP or his appointed representative (at least the project engineer). QT3 must be signed at site and later countersigned by Competent Person. QT3 must be kept and later compiled by Competent Person.
Periodic Site Visit Responsibility of the CP and will be emphasized in the conditions of approval.
Site supervisors must be qualified & duly appointed by the CP.
District should check periodically this on site and report to SPAN any deviations or work not supervised professionally.
2- QT4(a) (Pressure and Leakage Tests) Competent Person fill up QT1/Sixth Schedule to apply for pressure and leakage tests District notifies the date and time for the testing using standard notification letter QT2. QT4(a) will be filled and signed off at site by designated personnel. Development Unit at District must witness the test together with CP or his appointed representative (at least the project engineer). QT4(a) must be signed at site and later countersigned by Competent Person. QT4(a) must be kept and later compiled by Competent Person.
3- QT4(b) (PressureTest for Main Pipe Complete With Ferrule and Comm Pipe) Competent Person fill up QT1/Sixth Schedule to apply for pressure test District notifies the date and time for the testing using standard notification letter QT2. QT4(b) will be filled and signed off at site by designated personnel. Development Unit at District must witness the test together with CP or his appointed representative (at least the project engineer). QT4(b) must be signed at site and later countersigned by Competent Person. QT4(b) must be kept and later compiled by Competent Person.
4- QT5 (Water Tightness For Reservoir/Suction Cistern) Competent Person fill up QT1/Sixth Schedule to apply for water tightness test District notifies the date and time for the testing using standard notification letter QT2. QT 5 will be filled and signed off at site by designated personnel. Development Unit at District must witness the test together with CP or his appointed representative (at least the project engineer). QT5 must be signed at site and later countersigned by Competent Person. QT5 must be kept and later compiled by Competent Person.
5- QT6(a) (Final Joint Inspection – External Pipeline) Competent Person fill up QT1/Sixth Schedule to apply for final inspection District notifies the date and time for the inspection using standard notification letter QT2. QT 6(a) will be filled and signed off at site by designated personnel. Head Of District or his representative and Development Unit at District must witness the test together with CP or his appointed representative (at least the project engineer). QT6(a) must be signed at site and later countersigned by Competent Person. QT6(a) must be kept and later compiled by Competent Person.
6- QT6(b) (Final Joint Inspection – Reservoir/Suction Cistern/Pump House) Competent Person fill up QT1/Sixth Schedule to apply for final inspection District notifies the date and time for the inspection using standard notification letter QT2. QT 6(b) will be filled and signed off at site by designated personnel. Head Of District or his representative and Development Unit at District must witness the test together with CP or his appointed representative (at least the project engineer). QT6(b) must be signed at site and later countersigned by Competent Person. QT6(b) must be kept and later compiled by Competent Person.

Developer submit SYABAS' disclaimer statement letter and FORM G5 to District for Internal Plumbing
7- QT7 (Notification for Submission of BG/Bank Draft over DLP)
Head of district determines BG amount against defects and request for related document. District notifies Developer to prepare BG/Bank Draft for Defect Liability Period by writing a standard notification letter QT7.
8- QT8 (Support Letter from District O&M Dept for Tapping Work) Competent Person fill up QT1/Sixth Schedule to apply for tapping connection District notifies the date and time for the tapping connection using standard notification letter QT2. CP or his appointed representative (at least the project engineer). shall carry out the inspection for the tapping work together O&M District and certify that the tapping work is completed in accordance to approved drawings and Syabas standard and specification by signing off the QT8 at site. QT8 must be signed at site and later countersigned by Competent Person and Head Of District or Technical Manager.

Competent Person fill up QT1/Sixth Schedule to apply for Water Quality Testing. District notifies the date and time for the testing using standard notification letter QT2. Permit Holder, CP or his appointed representative (at least the project engineer) shall carry out the water quality testing together with WQ District and certify that the testing is completed in accordance with the requirements of the Water Services Industry (Water Reticulation and Plumbing) Rules 2014 and Syabas standard and specification by signing off the QT9 at site. QT9 must be signed at site and later countersigned by Competent Person and Head Of District or Technical Manager.
10 - QT10 Application For Handing Over Of External Water Reticulation System/Supply Main Developers prepare handing over documents and submit to District to hand over the External Water Reticulation System/Supply Main as explained in NINTH SCHEDULE (Subrule 29(1)) (according to Water Services Industry Rules 2014) Based on the declaration in QT10 the Consultant and Developer are fully responsible to ensure that the works have been carried out strictly in accordance with approved drawings and specification and using only approved material. In this regard we shall report to SPAN on the conduct of the consultant if found not acted professionally. District review the document for Approve Plan below 50,000 LPD and forward to Head Of Operation for approval. District review the document and forward to Take Over Section, HQ for Approved Plan 50,000 LPD and above to recommend for approval from Deputy Director
11- QT11 Acceptance Of Handing Over Of External Water Reticulation System/Supply Main
SYABAS District issues letter of Taking Over of External Water Reticulation System/Supply Main for < 50,000 LPD via TENTH SCHEDULE (Sub Rule 29(2)) (according to Water Services Industry Rules 2014) SYABAS HQ issues letter of Taking Over of External Water Reticulation System/Supply Main for equal or more than 50,000 LPD via TENTH SCHEDULE (Sub Rule 29(2)) (according to Water Services Industry Rules 2014)
Future Planning
Along these lines we intend to implement paperless strategies by adopting fully computerized system starting from approval stage up to taking over of the system. The Electronic Taking Over System will cover the scope be as follows: i.
Install network capabilities i.e. tool and software for site inspection efficiencies
ii.
Go live on an site inspection software for Taking Over
iii.
Perform security assessment and analysis data
As to show our commitment towards the above we are targeting to implement online application for all type of inspections and notifications for the tests in the first quarter of this year. We will tentatively schedule for the full digital application for taking over process in a very near future.
