1. Reference Blotter Entry No. 11-009 dated November 6 2015. 2. Please be informed that on or about 9:30 pm of this date that a shooting incident transpired at the Barangay Hall of Minante 1 Cauayan City Isabela where in the victim was identified as Roger Palma y Recto,35 years old, married, tricycle driver and a resident of Minante 2 Cauayan City Isabela, while the at suspect was identified as SPO2 MANUEL REYES Y PEDRO, 31 years old, married, presently assigned at La Union Police Station and a resident of Minante 1 Cauayan City Isabela. 3. Investigation conducted disclosed that on the said date,SPO2 MANUEL REYES Y PEDRO went to the house of ROGER PALMA to advice to stop that the loud music of videoke but Palma and his companion did not pay attention that resulted to misunderstanding wherein SPO2 Reyes said this statement “ Kapag di kayo titigil, babarilin ko kayo”. Therefore Roger Palma said that he will report to the barangay but SPO2 Reyes said “ hindi ako natatakot sa barangay! Magreklamo kayo!” Thus, Roger Palma went to the barangay hall to ask for assistance without knowing that SPO2 Reyes followed him ultered these word to quote against the victim “PAPATAYIN KITA WALA YAKA!” and shot Roger Palma in his head that caused his instant death. The suspect was arrested and recovered from his direct possession and control is one (1) 9mm beretta pistol with serial number ABC 12345 and one (1) magazine loaded with 12 live ammos and immediately brought to the police station for inquest proceeding. 4. AFFIDAVIT OF WITNESS I, NESTOR B ANDRES, 35 years old, married, barangay kagawad and presently residing at Minante 1, Cauayan City Isabela after having been duly sworn to in accordance with the law, do hereby deposes and state: That on November 6, 2015 at about 9:30 in the afternoon while conducted my duty on our Barangay Hall I saw the victim Roger R y Palma together with the suspect Spo2 Manuel Reyes y pedro living with the conversation ultered these words to quote against the former PAPATAYIN KITA WALANG YAKA!. Thereat I saw the victim shot by the said suspect and sustained gun shot wound on his head and was rushed to Cauayan Specialist Hospital but pronounced dead on arrival by Dr. Tomas Nicolas III attending physician. That I executed this affidavit/statement to attest the veracity of aforementioned facts and to support file charge/s in court against the suspect. Kagawad Nestor B. Andres of Minante 1, Cauayan City that SPO2 Manuel Reyes y Pedro, 31 years old, married, presently assigned at La Union Police Station and a resident of Brgy. Minante 1, Cauayan City shot Roger R. Palma in his head that caused his instant death with the used of calibre 9mm at the barangay hall of the said place. Upon receipt of the report, team led by the COP proceeded at the said place and upon arrival thereat, said team together with the said Brgy. Kagawad saw SPO2 Manuel Reyes y Pedro in the Barangay hall and arrested him. Recovered from his direct possession and control is one (1) 9mm Beretta pistol with serial number ABC 12345 and one (1) magazine loaded with 12 live ammos. Said suspect together with the recovered items was brought at Cauayan City Police Station.”

POLICE BLOTTER On or about 9:30 PM of November 06, 2015 a cellphone call received by this Police Station from Barangay Kagawad Nestor B Andres and reported that a shooting incident transpired at the Barangay Hall of Minante 1, Cauayan City Isabela. Upon receipt of the report, the team led by COP Venezuela proceeded at the said place and upon arrival thereat, said team together with the said Barangay kagawad saw the victim lying with his blood identified as Roger Palma y Recto, 35 years old, married, tricycle driver and a resident of Minante 1, Cauayan City, Isabela while the suspect identified as SPO2 Manuel Reyes y Pedro, 31 years old, married, presently assigned at La union Police Station and a resident of Barangay Minante 1, Cauayan City Isabela.
