PETITION COMES NOW, the petitioner and unto this Honorable Court most respectfully avers that1. He is of legal age, Filipino, single and a resident of Dona Carmen Homes, Mendiola street, Manila City; 2. He is a member of the Philippine Bar in good standing, having passed the Bar Examination given last October 2014 and admitted to the practice of law on April 5, 2015 with Roll of Attorney NO.12345; 3. He paid his Professional Tax (PTR) for the year 2015 with PTR No. 7654321 dated January 15, 2016 and Integrated Bar of the Philippines (IBP) membership for the year 2016 with IBP No. 12345678 dated January 15, 2016. Machine copies of the said receipt is hereto attach as ANNEX “A” and ANNEX “B” respectively; 4. He has no pending cases nor has been convicted of any offense/crime. Attached to this petition as ANNEX “C” is the Certification from the Honorable Executive Judge, as ANNEX “D” Certification from the Office of the Bar Confidant of the Supreme Court, as ANNEX “E”, the Certificate of Good Moral Character issued by Atty. Mauelito M. Manuel the incumbent IBP President for Manila City Chapter, and as ANNEX “F” the National Bureau of Investigation Clearance; 5. His three (3) passport size color photographs and a photograph of my office taken within thirty (30) days of the application, six (6) specimen signatures, and six (6) specimen imprints of the facsimile of his dry seal are herein attached and marked as Annexes “G”, “H”, “H” , “I” and “J” respectively and made an integral part of this petition;

6. He desires to be commissioned as Notary Public in and for the Manila City to end on December 31, 2017; 7. He possesses all the qualification and none of the disqualification for the office of the Notary Public. WHEREFORE, the undersigned petitioner most respectfully prays unto this Honorable Court that he be commissioned as Notary Public for the Manila City to expire on December 31, 2016.
