Interpretation
Patient LAA is an active business woman who sells sweet goods “ube.” As part of her daily routine she vends her product door-to-door in the community. She rarely visits the barangay health center for check-ups because she thinks that keeping herself healthy will prevent getting illness. Whenever she has a free day patient walks around their community for an exercise.
Patient LAA is now being hospitalized and consider she’s not healthy anymore; She was diagnosed with Community Acquired Pneumonia High Risk and had undergone confinement. She manifests fever prior to her confinement and difficulty of breathing. She is expecting to recover from his present condition with the help and support of her family and the health providers attending to her needs.
The onset of all pneumonias is marked by any or all of the following manifestations; manifestations; fever, chills, sweats, fatigue, cough, sputum production and dyspnea. (Medical-Surgical Nursing Vol. 2 8 th Edition by Joyce Black p.1599)
Patient LAA cannot function well with respect to his illness pneumonia evidenced by difficulty of breathing and fever. Patient also undergone oxygenation during confinement.

Patient LAA is on her low sugar diet consisting mainly of fruits, and vegetables. That’s being served in the hospital.

Nutritional and metabolic status of patient LAA has been changed slightly to her confinement and to her medical condition.
Elimination
Bowel Patient LAA defecates 1-2 times a week without experiencing discomfort. Stool is color brown and well-formed.
Bowel Patient LAA defecates daily. Stool is brownish in color, normal in amount but with discomfort.
Elimination is important because urinary and bowel movement affects your entire physiology. It makes the body pH in balance and it eliminates body toxins with it.

Bladder Patient LAA usually voids 5-7 times a day. Urine is light-yellow in color and without any discomfort.

Activity-Exercise
In the morning patient LAA walks around their community as an exercise. At her business, she usually sells door-to-door goods. After her work patient LAA mingles with her grandchildren. Patient LAA likes watching television shows, listening to radio and make conversation with her siblings.
Patient LAA’s activities in the hospital are limited only within her room. Patient LAA is activityintolerance due to her condition. She often lies in bed and only shifts to a chair with the help of her relatives whenever she wants to sit. She talks with her relatives whenever she feels boredom and frequently reads the newspaper.
