It is a hazardous event / incident which, under slightly different circumstances, could have caused an accident affecting people or an incident affecting property or the environment.
Safety Observation Card (SOC)
A safety observation system, which enables personnel to report both positive and negative safety issues and is meant to reduce the potential for injuries to themselves and others and to assist in reducing risk in the workplace. The system is based on SAIPEM CORPORATE “5 Stars Safety Intervention Tool”, which is an integral and fundamental part of the most efficient behavioural safety program (Leadership in Health & Safety – LiHS) SAIPEM has currently in place world-wide.

SCOPE OF DOCUMENT
This HSE Induction & Training Plan has been developed in line with CONTRACTOR and COMPANY HSEMS Requirements and Expectations, taking into consideration the specific needs of the project and the training topics implied by the onshore construction activities in KSA. Training programs, materials, topics, schedules and matrices have been reviewed and updated so as to fill, as far as reasonably practicable, all possible rooms for improvement. CONTRACTOR will provide training to workforce based on the onshore construction training needs identified. All training will be conducted in accordance with the project operations expectations and requirements. The prime scope of this HSE Induction & Training Plan is to: Provide required training to workforce based on their competence and trade in accordance with CONTRACTOR and COMPANY HSEMS Requirements. Prevent work-related injuries, occupational illnesses and environmental incidents. Provide required HSE training to all employees prior to starting their job. Promote continuous improvement in HSE performance and ensure that workforce have the required knowledge, skills and awareness, so as to enable them working in a safe, healthy and accident-free environment and environmentally sound manner. Encourage individual commitment to high standards of HSE Performance. Ensure that employees understand the potential of emergency situations and know what to do if something goes wrong in their area of work. Provide employees with information and feedback on lessons learned from previous incidents and exercises / drills. Define the topics to be addressed, as a minimum, for each trade. Provide a list of HSE Training topics which are to be given to all supervisors and workforce. Training will include competency assessment that demonstrates the knowledge and understanding of information transfer and learning outcome by trainees.

PROJECT OVERVIEW & BRIEF SCOPE OF WORK
Arabiyah & Hasbah Offshore and Onshore facilities are part of the WASIT Gas Program (WGP), which will produce approximately 2,500 MMSCFD of raw Khuff gas in order to support the KSA strategy to meet the increasing IK demand for sales gas. Processing Arabiyah and Hasbah gas is expected to produce 3,050 MMSCFD (1,450 MMSCFD for Arabiyah and 1,600 MMSCFD for Hasbah) of dry gas. The facilities to be fabricated and installed include new offshore gas production and Tie-in Platforms, one Intermediate Injection Platform, pipelines, both offshore and onshore, as well as the associated power supply and telecommunication system. Arabiyah and Hasbah are offshore fields located in Arabian Gulf, approximately 150 kilometers northeast of Jubail and 250 kilometers north of Dhahran. The fields lay in medium-depth waters in range of 40-60 meters. The new Arabiyah-Hasbah gas offshore platforms will be installed some 150-180 kilometers north east of the WASIT Gas Plant. Each of the single wellhead offshore platforms will connect to a field Tie-in Platform (one Tie-in Platform will be installed for each field). The gas from each field will be transported through dedicated subsea and onshore pipelines to the WASIT Gas Plant where new treating facilities will produce 1,700 MMSCFD of sales gas and 4200 MTD of sulphur.

A MEG Injection System is planned to transport continuously MEG and corrosion inhibitor from the onshore treatment plant to each Tie-in Platform to protect each trunkline against hydrates and corrosion. The corrosion inhibited MEG will be pumped offshore via an offshore pipeline to an IIP (ARBI TP-2) and then continues to ARBI TP-1 and HSBH TP-1 via two separate pipelines. Solvent Injection is required on each WHP upstream of choke valves to avoid the deposition of elemental sulphur along the production sealines. The Solvent Sulphur System will be actually designed for Hasbah Field only. However, future provision should be taken into account to allow the possibility of adding such facilities also to Arabiyah Field. The Scope Of Work (SOW) for Arabiyah & Hasbah Offshore and Onshore facilities shall include engineering, procurement of equipment and materials, fabrication, transportation, installation, hook-up, testing and precommissioning for the following:

5.
RESPONSIBILITIES
5.1
PROJECT DIRECTOR
The Project Director has the Overall and Final Responsibility for HSE and all project activities, including for authorizing the necessary HSE training to all project personnel, as needed. His responsibilities will be the following: Demonstrates visible, strong and proactive leadership and commitment towards HSE and does lead by example. Approves this HSE Induction & Training Plan and its future amendments and revisions. Allocates adequate human, technical and financial recourses for the correct implementation of this HSE Induction & Training Plan. Ensures that this HSE Induction & Training Plan is implemented throughout the work activities. Represents the Main Interface with COMPANY for all matters. Makes continuous efforts towards motivating CONTRACTOR and Subcontractor personnel to take an active part in HSE issues. 5.2
PROJECT MANAGER
The Project Manager has to ensure that all work activities under his control are carried out in accordance with CONTRACTOR and COMPANY HSE Policy. His responsibilities will be the following: Demonstrates visible, strong and proactive leadership and commitment towards HSE and does lead by example. Ensures that this HSE Induction & Training Plan is followed throughout his area of responsibility within the project activities and, implicitly, that relevant managers and supervisory personnel implement this plan within their area of work. Ensures that all necessary facilities and allowances are set to perform the training required for the project personnel, as needed. Provides support to the HSE Department, as and whenever needed. Ensures liaison with HSE Management for specialist advice. Ensures adequate HSE training of personnel to assure competency at work. Ensures that selected Subcontractors personnel have received adequate HSE training, within their organization, and their HSE training standards are in line with CONTRACTOR and COMPANY HSE requirements. Ensures that selected Subcontractors are monitored, throughout the work activities, to identify any additional training needs and to check that standards are maintained.

PROJECT HSE MANAGER
Project HSE Manager reports to the Project Director and will formally advise Project / Line Management with regard to health and safety at work and protection of the environment, including the relevant HSE training requirements as per project standards and local legislation. Project HSE Manager will liaise with SAIPEM CORPORATE and 3rd Party recognized training providers on any HSE training needs, as necessary. In addition to an advisory / consultancy role, he is responsible to: Audit and monitor, both internally and at Subcontractor level, the effectiveness of the HSE Training System and evaluate its performance. He will review this HSE Induction and Training Plan regularly and advise the Project Director and Project Management of any amendment for improvement. Liaise, on behalf of the CONTRACTOR, with appointed COMPANY Safety and Environmental Representative(s), as well as local government authorities or agencies. Review the effectiveness of the CONTRACTOR HSE performance and report to the Project Director. Prepare, monitor and periodically review HSE training programs / presentations. Ensure that all CONTRACTOR and Subcontractor employees receive HSE induction and specific HSE training. Evaluate the Subcontractors capabilities with respect to their management of HSE and ability to fulfill the specified requirements of the SOW. Monitor that emergency response drills are conducted as per defined emergency response drills schedule. 5.4
ONSHORE HSE MANAGER
Onshore HSE Manager reports to the Project HSE Manager with functional reporting to the Project Manager and assists other departments on HSE requirements. His responsibilities will be to: Liaise, on behalf of the CONTRACTOR Project HSE Manager, with appointed COMPANY Safety and Environmental Representative(s), as well as local government authorities or agencies. Measure the worksite performance of HSE issues on a regular basis using effective HSE performance criteria and indicators. Develop and prepare a complete HSE Induction and Training program for the supervisory level personnel and workforce. Provide Project HSE Manager with training information, hours and effectiveness. Keep training records such as training matrix, attendance list, objectives, duration, training topics and certification. Coordinate with the HR Department on necessary external training provided by 3rd Party recognized training providers. Evaluate the Subcontractors capabilities with respect to their management of HSE, training standards and ability to fulfill the specified requirements of the SOW. Assist Project HSE Manager with reporting the required statistical data to SAIPEM CORPORATE and COMPANY whenever necessary. Organize, in liaison with the Project Manager, emergency response drills according to the relevant emergency response drills schedule, and reports the outcome to the Project HSE Manager.

ONSHORE HSE COORDINATOR
The Onshore HSE Coordinator has a direct line reporting relationship with the Onshore HSE Manager, but also liaises with the Project HSE Manager as required. His responsibilities will be to provide full administrative and professional support, whenever needed, to the HSE induction and training activities and to maintain full records (training and related documentation), as requested by the Onshore HSE Manager. 5.6
HSE OFFICERS
The Onshore HSE Officers have a direct line reporting relationship with the Onshore HSE Coordinator, but also liaises with the Onshore HSE Manager as required. Their responsibilities will be to provide full administrative and professional support, whenever needed, to the HSE induction and training activities and to maintain full records (training and related documentation), as requested by the Onshore HSE Manager / Coordinator. They will also participate in TBTs, as required, and bring their contribution to the continual improvement of this important ‘on-the-job’ training tool. 5.7
DEPARTMENT (DISCIPLINE) MANAGERS / HEADS
Departmental management is responsible for actively demonstrating their commitment to the CONTRACTOR HSE Management System and for promoting SAFETY as part of their daily work activities. Specifically they will be responsible to: Demonstrate visible, strong and proactive leadership and commitment towards HSE and do lead by example. Ensure, throughout their areas of responsibility, that a safe working culture, where EVERYONE has both the authority and responsibility to STOP THE JOB if there is any doubt about the safety of the operation, is in place and adhered to at all times by all CONTRACTOR and Subcontractor personnel. This is to be emphasized and re-iterated during all HSE training sessions. Give full and pro-active support to the HSE department in identifying the training needs and requirements of their workforce. Ensure that all personnel under their supervision are fully trained and capable of undertaking work requirements, with respect to HSE elements of their work. Identify the HSE training needs of their workforce and notify the Project HSE Manager and the OnshoreHSE Manager accordingly. 5.8
SUPERVISORS
Supervisors are responsible to give full and pro-active support to the HSE induction and training activities, as instructed by their line / project management. 5.9
HR MANAGER
HR Manager will give full and pro-active support to the HSE Department, as required, with respect to the HSE induction and training activities (including needs for external training). 5.10 WORKERS All Personnel are responsible for carrying out their duties in a safe and environmentally sound manner, in accordance with the guidelines of the HSE Induction and Training received.

They are also responsible to attend and pro-actively participate in any HSE training scheduled and to acquire relevant knowledge to the greatest possible extent. Individuals must not undertake any task or operation for which they have not received adequate training or for which they lack proper instruction, tools, equipment or Personal Protective Equipment (PPE). 5.11 GENERAL RESPONSIBILITY Line Management, each in his area of work, are responsible for ensuring that their personnel receive HSE induction and relevant training prior to starting work. They are responsible to coordinate with the Project HSE Manager and/or the Onshore HSE Manager in order to facilitate conducting other appropriate HSE training courses in accordance with the project requirements and objectives. Immediately after recruitment, personnel will be scheduled, as necessary, for courses applicable to their trade to enable them perform their duties in line with the Project HSE Policy, CONTRACTOR and COMPANY HSE requirements, and this HSE Induction and Training Plan guidelines. All new arrivals will undergo HSE Induction training, prior to commencing work, according to the relevant project and worksite requirements. 5.12 SUBCONTRACTORS All Subcontractors must fully adhere to the Project HSE Policy, CONTRACTOR and COMPANY HSE requirements, and this HSE Induction and Training Plan guidelines. All Subcontractors must have in place and apply their own HSE Induction and Training System, so that to ensure and demonstrate, as needed, that all their personnel are qualified and trained as required. All Subcontractor personnel must be properly trained prior to commencing any work at any CONTRACTOR operated site / facility, so that to be able to meet CONTRACTOR and COMPANY HSE standards. If Subcontractors training standards are not up to the project requirements, then CONTRACTOR must give them additional HSE training before they start any job activity on the project. Subcontractor personnel training certificates and professional training courses conducted externally or internally will be recognized and the written evidence of the training and information will be provided on request to CONTRACTOR and COMPANY.
6.
TRAINING PROGRAMS
This HSE Induction and Training Plan is intended to encompass the HSE training requirements for all employees involved in the onshore construction activities and to ensure that Subcontractors are provided with necessary training in order to meet the Project HSE Policy, CONTRACTOR and COMPANY HSE requirements. Hence, CONTRACTOR has developed HSE training programs to comply with the relevant project HSE requirements and expectations. HSE training will be conducted only by qualified CONTRACTOR HSE personnel based in offices or at worksites. Whenever necessary, Project HSE Manager will request assistance from SAIPEM CORPORATE.

In case of HSE training topics / needs beyond CONTRACTOR and SAIPEM CORPORATE capabilities, 3rd Party recognized specialists or HSE Training providers will be contracted for training purposes. Although the official language of the project is English, communication barriers (such as for personnel whose mother tongue is not English) will be timely identified and the necessary provisions will be made to ensure that HSE trainings will be conducted with multi-lingual support, so as to enable full understanding of and maximum benefits from the training. 6.1
OFFICE HSE INDUCTION
The purpose of the office HSE induction is to provide adequate information on office related hazards and ensure that office based employees and visitors know what to do in emergency situations. Office visitors should receive HSE induction on their first visit. Office HSE Induction program to office employees will basically cover, but not be limited to, the following topics: Management and Employees Responsibilities / Project HSE goals and requirements Project HSE Policy Project Alcohol & Drugs Policy Project Smoking Policy Personnel access control to office Housekeeping Office Related Hazards – identifying, reporting and mitigation Emergency Response and Evacuation (routes to and location of Muster Point, DOs and DONTs in case of an emergency, alarms, fire extinguishers, emergency contact numbers) Expected behaviour in the office Environmental Awareness, including waste disposal Incident / Accident / Near Miss and Unsafe Practices / Conditions reporting protocol, including Safety Observation Card (SOC) Vehicles and Driving Policy / Mobile Phones Policy for DRIVERS / Use of Seatbelt in the Project Vehicles Manual Handling Awareness Medical Facilities, Services and Personnel Use of Stairways SUW (Stop Unsafe Work) – TOFS (Time Out For Safety) / Right To Refuse Unsafe Work Prior to starting the HSE Induction training, the HSE Personnel will issue the relevant Office HSE Induction form (refer to Attachment 1) to each new arrival, where they will fill in the relevant data and mark up the relevant boxes to confirm the scope of induction training they have received and understood. Then the Induction forms will be undersigned by both new arrival and the HSE Personnel. The HSE Personnel will maintain files of all these forms. 6.2
SITE HSE INDUCTION
The HSE Induction training will be performed on all CONTRACTOR, COMPANY, Subcontractors and 3rd Party new arrivals (for persons who have not been at site before) within the first day of arrival, except where, and based on objective reasons, time or availability of the Site HSE Personnel restricts this. In all instances, it should be carried out as soon as possible, as commencement of work and access to working areas of the site will be restricted until the HSE Induction is carried out. The HSE Induction training must be refreshed to all personnel who have left the site for a period longer than 6 months.

TOUR OF THE SITE
The Site HSE Personnel will take all new arrivals on a tour of the site. During this tour he will: Show escape routes and muster point(s). Show location of safety equipment such as fire extinguishers, fire hydrants, breathing apparatuses, spill response kits, etc. Show location of rest facilities, mess-hall, etc. Indicate specific hazards and precautions, including any restricted areas and who may go in them Give instructions on environmental protection and waste management / segregation / disposal. Further ask if there are any questions or problems and deal with them accordingly. 6.4

Tool-Box Talks (TBT) Slips, Trips & Falls Hand / Finger Injury Manual Handling Work at Height Management of Change Hazard Identification and Risk Assessment (HIRA), including Job Safety Analysis (JSA) Control of Substances Hazardous to Health (COSHH) Lifting Equipment and Operations Energy Isolation Permit to Work (PTW) System (including the specific training provided by COMPANY) Confined Space Entry (if applicable) Hot Work (e.g. welding, burning / flame-cutting, grinding, etc.) Operations Waste Management and Environmental Protection (*) Spill Response and Containment Methods Electrical Safety Excavation Safety Site Inspection Heat Stress Principles of Accident Prevention (Leadership in Health & Safety - LiHS) – conducted in close liaison with SAIPEM CORPORATE HSE Department. (*) As waste management is a very delicate and important aspect within the overall effective environmental protection process, the relevant training will cover, in depth, the following, but not limited to, items: CONTRACTOR and COMPANY requirements. The ‘Rule of 4 Rs’ – Reduce, Reuse, Recycle and Recover. Rules / good practices regarding minimization of waste generation. Waste classification. Waste segregation and its purpose and benefits. Procedures for waste monitoring, recording and tracking. Waste disposal, including COMPANY approved Subcontractors and different ways of treatment. The immediate job of preventing accidents falls upon the Supervisor not because it has been arbitrarily assigned to him but because accident prevention and production control are closely associated with supervisory functions. The most direct way to develop the desired attitudes and to impart the necessary information about HSE to Supervisors is to give them HSE training. The training course for Supervisors should include legal requirements, Project and Administrative policies and HSE aspects of the work likely to be undertaken by the Supervisors. Each training session will consist of the following steps: Presentation and discussion of relevant HSE topics through suitable training aids (e.g. videos, handouts ,etc) Practical exercises, whenever applicable. Issuing of training material / handouts to the attendants, whenever considered necessary. Assessment of the effectiveness of the training (course competency standards) by questioning the participants Recording of the training. The relevant In-House HSE Training Matrix is provided in Attachment 3. It is expected that those who have attended the recognized training courses have received all the necessary HSE instruction and information on the ‘duty-of-care’ concept and requirements, so that to carry out their day-to-day work activities safely and in full respect to the environmental protection. Whenever conducting HSE Training sessions, that should be correctly documented on the In-House HSE Training Attendance Sheet (refer to Attachment 4) and within the relevant In-House HSE Training Personal Record Sheet (refer to Attachment 5) and filed by the Site HSE Officers. 6.5
ONSHORE STATUTORY TRAINING / CERTIFICATION
All onshore heavy equipment and crane operators must be certified by COMPANY, in accordance with GI 7.025) to operate the specific heavy duty equipment to which they are assigned and must possess a valid relevant heavy equipment Saudi Arabian Government license, as required All Scaffold Supervisors and Inspectors must be certified by the COMPANY in accordance with GI 8.001. In accordance with GI 7.028, ALL Riggers must be certified by COMPANY as Rigger-III, Rigger-II, or Rigger-I for a maximum of two (2) years. The rigger certification identifies the load weight limits and type of lifts that can be rigged without supervision. COMPANY certified riggers will be used to rig ALL loads, regardless of weight to be lifted, configuration or location. This includes such operations as standard and critical lifts, loading or off-loading trucks. The following classification and restrictions will apply: Rigger-III - Can rig loads up to 10 tons (except loads involved in Critical Crane Lifts, covered in Section 5.0 of GI 7.028, that require a Rigger-I). Rigger-II - Can rig loads up to 40 tons, originate Critical Lift Plans, and rig / supervise crane suspended personnel platform (manbasket) operations. Rigger-I - Can rig all loads and approve Critical Lift Plans for critical lifts. Moreover, the following requirements must be adhered to whenever lifting operations are to be performed: A Rigger-III and / or a Rigger-II may only rig loads exceeding their certificate level if supervised by the Rigger of the higher certificate level.

Loads noted in approved Critical Lift Plans must be rigged by a rigger certified for that load category (Rigger-I, II, or III). However, all loads of 40 (forty) tons or greater require a Rigger-I to supervise the lift. The Rigger-I must physically examine the job site and the equipment to be used prior to the lift. He must be physically present to supervise all lifts that require a Rigger-I. All drivers must be in possession of a valid Saudi Arabian Government driving license. Whenever driving, all drivers must carry with them their driving license. 6.6
EMERGENCY TRAINING AND DRILLS
Emergency training and drills will be performed on a monthly basis in line with the emergency response drills schedule and will include but not limited to, the following topics: Site Evacuation (Muster Drill) Fire MEDEVAC Spill Emergency Response Drills will be reported on the form provided in Attachment 6. 6.7
COURSE COMPETENCY STANDARDS
Course Competency Standards are defined as goals or objectives to be personally achieved by a person during attendance of an HSE training course. On completion of the training, the minimum competency standards achieved by attendees will be, but not limited to, the following: Demonstrate a comprehensive understanding of the CONTRACTOR and COMPANY HSE Management System requirements. Demonstrate a full understanding of the applicable local legislation requirements. Define the HSE related responsibilities and accountabilities of themselves and others within the project operations and day-to-day work activities. Describe the methods of reporting an accident, incident, near miss and the structure of an emergency response, and actions an employee should take in the event of an emergency. Describe which type of fire extinguisher is suitable for extinguishing various classes of fire and demonstrate correct procedures. Understand the reasons and procedures for using the Permit to Work (PTW) System, the importance of safe working and the roles and responsibilities of persons working within the PTW System. Demonstrate a full understanding of the environmental protection and waste segregation and handling requirements. Demonstrate a full understanding of the health aspects relevant to the project works. 6.8

EXEMPTIONS
Where the specified course objectives or course competency standards are not considered to be appropriate to certain job categories of personnel, exemptions from the training requirements may be granted. For example, if an individual has extensive experience and can demonstrate equivalent HSE training related to this experience, then further training exposure may be unnecessary. In such circumstances, an exemption from HSE requirements may be granted: When an assessment of an individual’s job determines that the competencies to be delivered by the course are not appropriate to the work activities. When an individual can demonstrate a valid training certificate that covers the competency standards of the respective course, or provides other written evidence recognized by the Project / Onshore HSE Manager.

AUDIT & PROGRAM REVIEW
The Project HSE Manager will audit and monitor, both internally and at Subcontractor level, the effectiveness of the HSE Training System and evaluate its performance. He will review this HSE Induction and Training Plan regularly and recommend to the Project Director and Project Management any amendment / action deemed necessary for improvement.
