POTENTIAL HAZARDS

Refer HSEMS Procedures - Doc. No. SA.KOC.004 “Permit to Work” and SA.KOC.020 “Plant Safety Override Registration”.



Take a Cold Work Permit from Asset Owner.



Refer manufacturer instruction for functional testing & CO2 Flooding System.



Asses the risk & Override the Fire Shutdown & CO2 actuation of the zone where CO2 Flooding System is to be tested.



Do not remove override prior to resetting the alarm in the panel.



Plan the work involving personnel responsible for preparation of CO2 Flooding System functional testing.



Conduct Tool Box Talk.



Maintain a radio communication between worker at CO2 Flooding System location & person stationed near F & G panel / Control Room.

Accidental Release of CO2



Close the isolation valve on CO2 Cylinder Bank Manifold or isolate CO2 bottles individually before function testing.

CO2 Exposure / Asphyxiation



Ensure no body is inside the enclosure for which CO2 Flooding System is to be function tested.

Defective Appliances



Keep the spare appliances (such as pneumatic switch, actuator etc) for replacement in case found defective.



Ensure CO2 Cylinders are not leaking.



Replace empty CO2 Cylinder.

Fault Alarm



Confirm the alarm through radio communication; whenever CO2 Flooding System is function tested.

Leaking Discharge Nozzles



Ensure the closure of pneumatic switch / actuator before lining up CO2 Cylinder Bank after completion of functional testing.

Failure to Reset the Fire Alarm



Do not line up CO2 Cylinder prior to resetting the fire detection alarm on F & G panel.



Reset the fire detection alarm at F & G panel.



Remove the Fire Shutdown override after evaluating the risk.



Ensure proper wind up & housekeeping at worksite.
