I.GENERAL SAFE PRACTICES The safe practices described in this book are for your protection. Read and observe them. Accidents happen without warning and many are caused by lack of knowledge, inattention and thoughtlessness. Lack of knowledge of these safe practices will not justify noncompliance.

Check your work area to determine what problems or hazards may exist.

Your activity may endanger fellow workers or nearby equipment and materials. Take the necessary steps to safe guard them.

3.

Review the safety requirements of each assigned job with your supervisors.You will not be expexted to do a job which might result in injury to your self or others.

4.

Be sure you understand emergency instructions. Anticipate what you will do in case of an emergency. Above all, be calm

5.

Don’t wear jewelry (rings, bracelets, or neck chains) on the job

6.

Report unsafe equipment, hazardous conditions, and unsafe acts to your supervisors. Use safety equipment when it is specified for the job. Safety equipment is available through the tool room or the safety office

Practice good housekeeping in your work area. Failure to follow practices relating to your safety or that of fellow employees or failure to safeguard equipment, tools, or materials properly may lead to your discharge.

I.PRAKTEK KESELAMATAN KERJA SECARA UMUM Cara-cara bekerja yang aman yang diuraikan didalam buku ini adalah untuk perlindungan anda. Bacalah dan ikutilah semua ini .Kecelakaan terjadi tanpa diduga, seringkali disebabkan oleh kurangnya pengetahuan, tidak hati-hati atau ceroboh. Kurangnya pengetahuan mengenai cara-cara kerja yang aman ini tidak dapat dijadikan alas an bila terjadi kesalahan. 1. Periksalah tempat kerja anda untuk menentukan persoalan2 atau bahaya2 yang sekiranya dapat timbul 2. Kegiatan anda mungkin membahayakan teman2 sekerja anda atau membahayakan alat2 dan benda2 didekat anda. Ambilah tindakan2 penyelamatan yang diperlukan. 3. Periksa kembali persyaratan2 mengenai keselamatan kerja untuk setiap pekerjaan yang ditugaskan supervisor pada anda. Anda tidak akan diminta untuk mengerjakan suatu pekerjaan yang mungkin dapat menimbulkan luka pada diri anda atau orang lain. 4. Anda harus tahu tentang perintah2 dalam keadaan bahaya.Pikirkan lebih dahulu apa yang harus anda perbuat bila anda dalam keadaan bahaya. Yang terpenting adalah anda harus tetap tenang 5. Jangan memakai perhiasan (cincin, kalung dan gelang) dipekerjaan. 6. Laporkanlah alat2 yang tidak baik, keadaan2 yang membahayakan kepada pengawas anda 7. Pakailah alat2 keselamatan yang diharuskan dipakai untuk suatu pekerjaan . Alat2 keselamatan tersebut tersedia digudang atau dikantor keselamatan kerja. 8. Tata cara kerja & housekeeping yang baik di area tempat kerja 9. Kegagalan mengikuti cara bekerja yang menyangkut keselamatan anda, atau keselamatan orang lain, kurang baik memelihara alat2, perkakas, atau bahan2, dapat mengakibatkan pemecatan anda.

II.OFFICE SAFETY Office are comparatively safe places in which to work, yet accidents occur. To make your work area safe .Follow these rules :

II.KESELAMATAN KERJA DI KANTOR Kantor adalah tempat yang dapat dikatakan aman untuk bekerja, namun demikian sering juga terjadi kecelakaan.Agar tempat anda aman, ikutilah peraturan2 dibawah ini : 1.

Be careful of swivel chairs. Do not slump back in them without first testing your weight gradually. Be sure you have a firm footing when you have to climb. Use a stepladder

Walk, do not run, in corridors or in stairs. Use handrails.

Check office furniture regularly for sharp edges or splinters and loose casters or bolts.

Hati-hatilah dengan kursi putar. Jangan merebahkan diri dikursi tersebut sebelum menguji berat anda dengan pelan-pelan. Pastikanlah bahwa kaki anda mempunyai alas berpijak yang kokoh bila anda hendak memanjat. Gunakanlah jenjang yang baik. Berjalanlah, jangan berlari digang atau ditangga gedung. Berpeganglah pada susuran tangga. Jangan berdiri dan bercakap-cakap didepan pintu tertutup, pintu-pintu tersebut dapat dibuka secara mendadak. Janganlah dorong-mendorong atau berjejal-jejal di lift, pintu masuk, atau ditangga gedung Bacalah surat-surat dan bacaan-bacaan lain dimeja anda, jangan membaca sambil berjalan-jalan Perhatikanlah kabel-kabel telepon, kabel mesin-mesin kantor, keranjang-keranjang sampah dan lain-lain yang berbahaya dibawah kaki anda, yang dapat menjatuhkan anda. Pakailah pegangan pada waktu menutup almari file, laci2 meja, dan pintu lemari besi dan pintu ruangan yang aman dibawah tanah. Periksalah perabot kantor secara teratur, kalau ada sudut yang tajam atau serpihan2 yang runcing atau ada roda2 atau baut2 yang kendor Simpanlah benda2 tajam ditempat yang aman. Pergunakanlah benda2 tersebut dengan hati2. Periksalah dengan baik agar mesin2 tik / computer ditaruh ditempat yang kuat Jangan menyetel atau membersihkan mesin2 kantor yang dijalankan dengan listrik pada saat mesin2 tersebut sedang jalan. Jangan mencoba memperbaiki sendiri alat2 listrik. Panggilah tukang yang ahli. Bila anda diizinkan merokok, pakailah tempat abu. Patuhilah tanda “Dilarang Merokok”. Laporkanlah segera semua luka2/sakit yang terjadi ditempat pekerjaan kepada pengawas anda dan kepada pertolongan pertama.

III.HOUSEKEEPING Good housekeeping is an important part of our safety program. It is the responsibility of all employees, supervisors and craftsmen alike to practice good housekeeping. 1. Scrap materials and rubbish are fire and accident hazards. If an excess of these materials exists in your work area, ask your supervisor to arrange for their removal. 2. You must use the trash barrels which are located throughout the jobsite, if you need one in your immediate work area, notify your supervisor. 3. Return all surplus materials to the stockpile at the completion of your job. 4. Do not leave tools and materials where they will create a hazard for others. Put them in the gang box or return them to the tool room. 5. Place oily rags in approved metal containers. 6. Wipe up spilled liquids immediately. If you can’t handle the problem, notify your supervisor so that he can arrange for the necessary clean up. 7. Keep change room clean. Do not let soiled clothes, food scraps, and soft drink bottles accumulate. If drinking cups are used, deposit them in the containers provided. Also place sandwich wrappers, paper bags and other trash in the container. 8. Toilets, wash up facilities and water dispencers are provided for your convenience and comfort. Please help to keep them clean and sanitary. Report any problems to your supervisor.

III.HOUSEKEEPING Memelihara tempat pekarjaan dengan baik merupakan bagian yang penting dalam program keselamatan kerja. Adalah tanggung jawab semua pekerja, pengawas2 dan tukang2 untuk memelihara tempat kerja dengan baik. 1. Bahan2 yang tidak terpakai dan sampah merupakan bahaya kebakaran dan kecelakan. Bila bahan2 yang tidak terpakai ini berserakan diruang kerja anda, mintalah kepada pengawas anda untuk mengusahakan pembuangannya. 2. Pakailah tong2 sampah yang disediakan diseluruh tempat kerja. Bila anda memerlukan tong sampah untuk ditaruh didekat anda, beritahukanlah kepada pengawas anda. 3. Kembalikanlah semua bahan2 yang berlebihan ditempat penyimpanannya, setelah anda selesai bekerja. 4. Dilarang membiarkan perkakas2 dan barang2 terletak ditempat yang membahayakan orang lain.Taruhlah barang2 tersebut didalam kotak besi atau kembalikanlah ke gudang perkakas. 5. Taruhlah robekan2 kain yang berminyak ke dalam tong besi. 6. Cairan2 yang tumpah harus segera dibersihkan. Jika anda tidak bisa mengerjakannya beritahukanlah hal ini kepada pengawas anda, supaya dia bisa mengusahakan pembersihan seperlunya. 7. Kamar ganti pakaian harus dijaga agar tetap bersih. Pakaian2 kotor, sisa2 makanan, dan botol2 minuman jangan sampai menumpuk. Kalau untuk minum dipakai cangkir2 kertas buanglah cangkir2 tersebut ke dalam tong sampah yang tersedia. Demikian juga buanglah pembungkus2 roti, kantong2 kertas, dan sampah2 lainnya ke dalam tong sampah itu. 8. Toilet, tempat mencuci, tempat minum disediakan agar anda merasa senang dan puas. Jagalah agar tempat2 tersebut tetap bersih dan memenuhi syarat2 kesehatan. Laporkanlah setiap masalah kepada pengawas anda.

9. Selection of suitable materials for floors and floor covering for Construction departments id of vital importance as many accidents are caused by slips and falls. Any floor surface however , is hazardous following spillage of grease or other liquids. The basic rule is that spillage of any kind should be cleaned up immediately it occurs. If this cannot be done, the spillage must be give sign 10. Use of correct cleaning materials for a particular surface is of prime importance as the application of incorrect agents can destroy the non slip properties of any surface and in the case of quarry tiles damage the grouting between them. Construction managers should agree cleaning material and techniques with their domestic service colleagues 11. Good lighting standards in all areas of a Construction department is vitally important as dark corners or heavy shadow can give rise to cuts or slips 12. Broken or missing bulbs / tubes should be reported immediately and replace – ments made as quickly as possible. Regular cleaning is also important to maintain standards

9. Pemilihan bahan-bahan yang sesuai untuk lantai dan penutup lantai bagi department Construction adalah sangat penting karena banyak kecelakaan disebabkan oleh licin & jatuh.. Permukaan lantai bagaimanapun merupakan berbahaya karena licin disebabkan minyak atau cairan lainnya. Aturan yang mendasar adalah bahwa kelicinan dari apa saja mestilah segera dihilangkan atau dibersihkan. Jika hal ini tidak bisa dilakukan lantai yang licin itu mestilah ditandai. 10. Pemakaian bahan2 pembersih yang tepat untuk suatu permukaan khusus adalah penting karena pemakaian zat2 yang tidak tepat bisa merusak zat2 non slip dari permukaan lantai dan dalam hal ubin berkerikil/kesat menjadi rusak. Manager Construction haruslah sependapat dengan teman sekerjanya mengenai cara2 dan bahan2 pembersih itu. 11. Standar penerangan yang bagus dalam semua ruangan didepartemen Construction adalah sangat penting karena sudut-sudut yang gelap atau bayangan yang gelap bisa meningkatkan terjadinya luka dan slip 12. Bola lampu yang putus atau kosong haruslah dilaporkan dengan segera dan penukarannya haruslah secepatnya dilakukan. Pembersihan yang regular penting untuk menjaga standard


IV. PERSONAL PROTECTIVE EQUIPMENT Personal Protective Equipment is available for your use. When equipment has been specified for certain work assignments or areas, you must use it. 1. All employees, visitors, must wear a hard hat in designated areas 2. You must wear clothing suitable for the work you are doing. Minimum attire is long pants and a T-shirt. 3. Wear sturdy work shoes, safety shoes are desirable. Sneakers and sandals are not permitted. 4. You must wear proper eye protection when exposed to flying objects, dust, chemicals, or harmful rays. 5. Hearing protective equipment is available upon request. You may be required to use it in designated areas or for specific jobs. 6. Respiratory protective equipment may be required in areas where health hazards exist due to accumulations of dust, fumes, mists or vapors. 7. Safety belts and lifelines must be used when other safe guards, such as nets, plinking, or scaffolding cannot be used. Be sure safety lines are independent of other rigging 8. Wear gloves when handling objects or substances which could cut, tear, or burn the hands. 9. Your must wear rubber boots for work in concrete, mud or water. 10. Hair nets may be required for employees whose hair is a potential source of injury. 11. Electricians using insulated gloves must test them for defects daily. 12. Fall protection a. Appropriate fall protection is required whenever working at heights that have potential injury for falling b. Appropriate selection of correct fall protection for each individual situation is required.

IV. ALAT PELINDUNG DIRI Alat2 untuk melindungi tubuh anda tersedia untuk dipakai. Jika alat2 untuk dipakai pada suatu pekerjaan atau suatu tempat kerja telah ditentukan maka anda harus memakainya. 1. Semua pekerja, pengunjung, harus memakai helm ditempat yang telah ditentukan. 2. Anda harus berpakaian sesuai dengan pekerjaan yang sedang anda laksanakan .Minimal anda harus memakai celana panjang dan baju oblong. 3. Pakailah sepatu kerja yang kokoh. Sepatu pengaman lebih baik lagi. Sepatu karet dan sandal tidak diperbolehkan. 4. Anda harus memakai pelindung mata yang cocok bila menghadapi benda2 yang beterbangan, debu, bahan2 kimia,atau sinar2 yang merusak mata. 5. Alat pelindung pendengaran dapat diberikan atas permintaan. Anda mungkin diharuskan untuk memakainya ditempat2 yang telah ditentukan atau untuk pekerjaan2 khusus. 6. Alat penolong pernafasan mungkin diharuskan dipakai ditempat2 dimana ada bahaya2 yang dapat mengancam kesehatan, sebagai akibat dari debu yang terkumpul, asap, kabut atau uap. 7. Sabuk pengaman penolong dan alat2 penolong harus dipakai jika alat pengaman misalnya jala2, papan atau perancah, tidak dapat dipakai. Periksalah baik2 apakah alat2 pengaman tersebut terlepas dari tali temali lain. 8. Pakailah sarung tangan bila anda memegang benda2 atau zat2 yang dapat memotong, merobek, atau membakar tangan. 9. Anda harus memakai sepatu boot karet bila bekerja ditempat beton, Lumpur, atau air. 10. Jala rambut harus dipakai oleh pekerja2 yang rambutnya merupakan sumber kemungkinan mendatangkan malapetaka. 11. Tukang2 listrik yang memakai sarung tangan berisolasi harus memeriksanya setiap hari kalau2 ada yang rusak. 12. Pelindung terhadap bahaya jatuh : a. Alat pelindung diri dari bahaya jatuh harus dikenakan bila ada kemungkinan cidera jatuh b. Peralatan pelindung jatuh harus sesuai dengan situasi yang dihadapi.


V. PERTOLONGAN PERTAMA

Fasilitas2 pertolongan pertama disediakan untuk

First Aid facilities are provided for your safety. Qualified personnel are available to render treatment and to maintain required records. 1. Report all injuries immediately, no matter how minor, to your supervisor and to first aid.Treatment will be given and the incident will be recorded. Should later medical care be needed, you will have fulfilled your obligations. 2. Drug, tranguilizers and insulin must not be taken on the job unless authorized in writing by your personal physician. A copy of thie authorization should be in your first Aid file 3. First Aid teams are organized and trained to render assistance. In the event of an injury or illness you will get proper first aid treatment. Your physician will be notified in the event of an emergency 4. If you had military, red cross or other First aid or medical training tell the job safety representative. You may be invited to join the project First aid team. 5. Never move an injured or seriously ill person unless necessary to prevent further injury. Emergency steps for notifying First aid are posted throughout the jobsite, familiarize your self with them. First aid should not be administered by non designated employees except in case of severe bleeding or cessation of breathing

Laporkan semua luka2 dengan segera, tidak peduli betapa kecilnya, pada pengawas anda dan pada pertolongan pertama. Pengobatan akan diberikan dan kejadian itu akan dicatat. Bila kemudian diperlukan perawatan medis anda telah memenuhi kewajiban anda. Obat2an, obat penenang, dan insulin tidak boleh diminum ditempat pekerjaan, kecuali jika diijinkan secara tertulis oleh dokter anda.Salinan dari ijin ini harus disimpan didalam arsip anda di pertolongan pertama. Team pertolongan pertama diorganisir dan dilatih untuk memberi pertolongan. Bila terjadi luka2 / sakit anda akan mendapatkan perawatan pertama yang layal. Dokter anda akan diberitahu bila terjadi keadaan mendadak. Bila anda pernah mendapatkan latihan pertolongan pertama atau pelajaran2 mengenai kedokteran diketentaraan atau dipalang merah, beritahukan kepada petugas Keselamatan Kerja. Mungkin anda diminta untuk turut serta dengan team pertolongan pertama . Jangan sekali-kali memindahkan orang yang sedang sakit keras atau luka2 untuk mencegah luka2 yang lebih parah lagi.Tindakan2 darurat untuk memberitahu pada pertolongan pertama telah dipasang diseluruh tempat pekerjaan.Pelajarilah betul2 cara tersebut. Pertolongan pertama hendaknya jangan diberikan oleh karyawan yang tidak ditunjuk khusus untuk keperluan itu, kecuali jika terjadi pendarahan yang keras atau terhentinya pernafasan.

BURNS If an accident occurs in the work place in your work location and it causes burns because of fire, hot (boiled) water or electricity or chemical substances, you must give first aid to the patient such as : 1. To prevent shock 2. To make pain less 3. To prevent Infection

To prevent shock Most of accident injuries are followed by shock .  Put patient at rest  Keep the patient quiet and warm  Loosen tight clothing  Keep the patient calm and confident of receiving help quickly

To make the pain less You may put the burn pat of body into clean, cold water / pipe water. You may also compress the burns instead using a towel that has been watered with cold water. Do it until the pain vanishes, lasting for 30 minutes to 5 hours

Minor Injuries Althoungh the injury is little, you should report it to your supervisor. All of the injuries can become infected and turn septic if not treated promptly

Food Poisoning It is an illness that is caused by eating contaminated food. This illness can lead to death

What are the sysptons ?    

Vomiting Diarrhoea Abdominal pain Fever

They last between one and seven days

Kalau terjadi kecelakaan ditempat kerja dilokasi saudara misalnya mengakibatkan luka bakar karena api, air panas, atau arus listrik , zat2 kimia saudara harus memberikan pertolongan pertama berupa : 1. Mencegah shock 2. Mengurangi rasa sakit 3. Mencegah infeksi

Mencegah Shock Kebanyakan cidera kecelakaan diikuti oleh shock (kegoncangan jiwa)  Usahakan agar si pasien tenang  Usahakan agar pasien merasa damai dan hangat  Jika pakaiannya ketat harus dilonggarkan  Yakinkan si pasien bahwa bantuan akan datang dengan cepat

Mengurangi rasa sakit Dengan merendam bagian yang terbakar dalam air ledeng yang bersih, dingin, atau air es. Dapat juga dilakukan dengan mengompresnya dengan handuk yang direndamkan ke air es. Lakukanlah hal ini sampai rasa sakit itu hilang, berlangsung antara 30 menit sampai mungkin 5 jam

Luka Ringan Walaupun luka itu kecil, haruslah dilaporkan kepada atasannya. Segala luka bisa menjadi infeksi dan akan busuk, jika tidak mendapatkan pengobatan.

Keracunan Makanan Keracunan makanan adalah suatu penyakit karena memakan makanan yang tercemar, yang dapat menyebabkan kematian. Apa saja tanda-tandanya ?  Muntah-muntah  Sakit perut / mencret  Perut kejang  Demam Gejala ini berlangsung 1-7 hari

VI. ELECTRICAL All temporary electrical equipment used on the jobsite will be listed by an approved testing laboratory for the specific application all temporary electrical installations must conform to the national electric code. 1. All electrical tools and equipment must be grounded. 2. Damaged or defective electrical tools must be returned immediately to the tool room for repair 3. Electricians are the only employees authorized to repair electrical equipment.Tampering with tools or equipment may result in an employee’s discharge. 4. Temporary lighting used in damp and / or hazardous locations must be operated at a maximum of 12 volt. 5. You must not work on or in proximity to energized circuits of any voltage unless adequate safety measures have been taken and the work operation has been reviewed and approved by the electrical superintendent 6. When it is necessary to work on energized lines and equipment, rubber gloves, blankets, mats, and other protective equipment must be used. 7. Temporary electric cords must be covered or elevated. They must be kept clear of walkways and other locations where thwy may be exposed to damage or create tripping hazards. 8. Splices in electrical cords must retain the mechanical and dielectric strength of the original cable. 9. Temporary lighting must have guards over the bulbs. Broken and burned out lamps must be replaced immediately. 10. Energized wiring in junction boxes, circuit breaker panels, and similar places must be covered at all times. 11. Hazardous areas must be barricaded and appropriate warning signs posted.

VI. PERLISTRIKAN Semua alat2 listrik yang dipasang secara sementara ditempat pekerjaan akan didaftar oleh laboratorium penguji yang telah disetujui untuk penggunaan khusus tersebut.Semua instalasi2 listrik sementara harus sesuai dengan code listrik nasional. 1. Semua perkakas dan alat2 listrik harus dihubungkan dengan tanah 2. Perkakas2 listrik yang rusak atau cacat harus dikembalikan dengan segera ke gudang perkakas untuk diperbaiki. 3. Tukang 2 listrik adalah pekerja yang hanya dibenarkan untuk memperbaiki alat2 listrik. Mencoba2 memperbaikinya dengan perkakas2 atau alat2 dapat berakibat seorang karyawan dipecat. 4. Bila harus bekerja dibagian2 yang beraliran listrik, sarung tangan karet, selimut, tikar dan alat pelindung lainnya harus dipakai. 7. Kabel2 listrik sementara harus tertutup atau dipasang tinggi. Tidak boleh merintangi jalanan dan tempat2 lainnya, karena kalau demikian kemungkinan besar kabel2 tersebut akan rusak atau menimbulkan bahaya tersandung kaki. 8. Sambungan2 kabel listrik harus dibuat sedemikian sehingga tetap memiliki kekuatan2 mekanis dan dielektris dari kabel yang asli. 9. Penerangan listrik sementara harus mempunyai pelindung yang menutupi lampu2 pijarnya. Lampu2 pijar yang putus atau terbakar harus diganti dengansegera. 10. Kawat2 didalam kotak2 penyambung panel2 pemutus aliran dan tempat2 lain yang sejenis yang beraliran listrik harus selalu tertutup. 11. Ditempat2 yang berbahaya harus dipasang rintangan2 dan tanda2 peringatan sebagaimana mestinya.

VII.PERKAKAS2 BIASA DAN PERKAKAS2 LISTRIK YANG MUDAH DIBAWA Seorang tukang dinilai dari kondisi perkakas2nya dan cara memakainya. Hanya perkakas2 yang dalam keadaan baik akan dikeluarkan dari gudang perkakas. Anda harus mengindahkan semua petunjuk2 dari pabrik pembuatnya. Selain daripada itu ikutilah cara2 kerja yang aman sebagai berikut ini :

UMUM 1. Periksalah perkakas2 anda setiap hari untuk memastikan apakah perkakas2 tersebut dalam keadaan baik, sebagaimana mestinya. Perkakas2 yang rusak atau cacat harus dikembalikan ke gudang perkakas. 2. Gergaji2 bertenaga listrik, gerinda2 dan perkakas2 bertenaga listrik lainnya harus selalu dilengkapi dengan bagian2 pengaman. Mencabut bagian2 pengaman itu atau membuatnya tidak berguna lagi dapat merupakan alas an untuk memecat anda. 3. Perkakas2 bertenaga listrik harus diangkat atau diturunkan dengan tali yang diikatkan pada badannya, dan sekali-kali tidak boleh dengan kabelnya atau selangnya. 4. Kabel2 dan selang2 harus dijauhkan dari jalanan orang, dari tangga gedung dan tangga2 biasa. 5. Bila menggunakan perkakas2 dalam daftar dibawah ini, atau bekerja berdekatan dengan orang lain yang menggunakan perkakas2 tersebut, anda harus memakai alat pelindung tubuh tambahan sebagai yang diperincikan dibawah ini. VIII.MATERIAL HANDLING, STORAGE & DISPOSAL All material must be properly stacked and secured to prevent sliding, falling or collapse. Aisles, stairs, and passage-ways must be kept clear to provide for the safe movement of employees and equipment and to provide access in emergencies. 1. Use proper lifting techniques when handling materials : a. Get down close to the load b. Keep your back straight c. Lift gradually, using your legs d. Do not jerk or twist e. Get help for bulky or heavy loads 2. Stored materials must not block any exit from a building 3. Material stored inside buildings or structures under construction must not be placed wihin 6 feet of any hoistway or other inside floor opening, nor within 10 feet of any exterior wall which does not extend above the top of the material stored. 4. Pipe, conduit, and bar stock should be stored in racks or stacked and blocked to prevent movement. 5. The quantity of materials stored on scaffolds, platforms, or walkways must not exceed that required for 1 day operation 6. Material must never be thrown or dropped from a distance of more than 20 feet.The drop area must be barricaded to protect personnel from being struck by falling materials. Thrash chutes are required for droping materials from heights above 20 feets. 7. Protruding nails must be bent or pulled when stripping forms or uncrating materials.

VIII.CARA MENANGANI MENYIMPAN & MEMBUANG BARANG Semua barang2 harus disusun dengan baik dan dikokohkan supaya tidak merosot, jatuh atau runtuh. Gang2, tangga2, gedung dan jalanan harus dijaga agar selalu bersih, supaya karyawan dan alat2 dapat bergerak dengan aman dan bisa merupakan jalan keluar / masuk dalam keadaan darurat. 1. Pakailah teknik mengangkat yang baik jika menangani barang2: a. Berjongkoklah didekatbarang yang akan diangkat b. Tegak luruskanlah selalu punggung anda c. Angkatlah pelan2 dengan menggunakan kaki anda. Jangan menyentak atau berputar d. Mintalah bantuan untuk mengangkat barang2 yang besar / berat. 2. Barang2 yang disimpan tidak boleh menghalangi pintu keluar mana saja dari gedung 3. Barang2 yang disimpan didalam gedung2 atau bangunan2 yang sedang dikerjakan tidak boleh diletakkan dalam jarak 6 kaki dari tempat naiknya barang, atau dari lobang lantai lainnya yang ada didalam, tidak boleh pula dalam jarak 10 kaki dari tembok luar yang tidak setinggi barang2 yang ditumpuk 4. Pipa2, pipa2 listrik dan batangan2 yang disimpan harus ditaruh dirak2 atau ditumpuk dan diberi penahan agar tidak dapat bergerak. 5. Jumlah barang2 yang ditaruh diatas lantai / jalanan tidak boleh melebihi jumlah persediaan untuk satu hari kerja. 6. Barang2 tidak boleh dilempar atau dijatuhkan dari tempat ketinggian lebih dari 20 kaki. Tempat jatuhnya harus diberi penghalang supaya karyawan2 tidak tertimpa barang2 yang sedang dijatuhkan. Peluncur2 pembuang sampah harus disediakan untuk menjatuhkan benda2 dari ketinggian diatas 20 kaki. 7. Paku2 yang menonjol harus dibengkokkan atau dicabut pada waktu membuka / membongkar barang2 dari peti barang.

XI. INCIDENT REPORTING It is the policy of the company that all incident resulting in injury, damage to equipment, or damage to property, no matter how minor, must be immediately reported to the supervisor. All incident include “nearmiss” are to be thoroughly investigated and the necessary remedial actions taken. In the case of a fatal accident, heavy accident, or accident with lost days: a Ensure that all victims received medical treatment. b Ensure that the scene of the accident is not tampered with and the necessary security has been made. c Advise the said accident to the direct supervisor concerned, HSE Department and Construction Supervisor. d Inform the HSE Department of matters required to prepare a preliminary report. e Carry out immediately preventive actions so that the accident will not happen again until the accident is completely investigated and rectification actions are made. f Carry out investigations at the location, complete the accident report forms and send them to the HSE Department soonest. In the case of a light accident: a

Ensure that all victims received medical treatment, and required security measures have been made to all involved. b Advise the accident to the HSE Department and the direct supervisor concerned. c Complete the accident investigation form and send it to the HSE Department. d Carry out rectification in order to prevent reoccurrence of the accident. In the case of incident / near miss: a.

Ensure that activities at the work place where the incident / near miss occurred is stopped to prevent an accident from happening. Immediately discuss the situation, know the type of action to required and furthermore carry out rectification. Make an incident report, state what actions were taken so that the situation truly became safe and discuss with the direct supervisor.

XI. LAPORAN KECELAKAAN Perusahaan mempunyai kebijaksanaan bahwa semua kecelakaan yang berakibat cidera, kerusakan alat atau barang perusahaan seberapapun kecilnya harus segera dilaporkan kepada supervisor. Semua kecelakaan termasuk “nearmiss” akan diinvestigasi secara menyeluruh dan tindakan perbaikan yang diperlukan harus dilakukan Dalam hal kecelakaan fatal, berat atau kecelakaan dengan hari hilang : a. Pastikan bahwa semua korban telah mendapatkan perawatan medis. b. Pastikan bahwa lokasi terjadinya kecelakaan tidak diubah dan telah dilakukan c. Beritahukan kecelakaan tersebut kepada pengawas langsung yang bersangkutan, HSE Departemen dan Supervisor d. Informasikan kepada HSE Departemen hal – hal yang diperlukan untuk membuat laporan pendahuluan. e. Lakukan tindakan pencegahan dengan segera agar kecelakaan tidak terjadi lagi sampai kecelakaan tersebut diselidiki dengan sepenuhnya dan rencana tindakan perbaikan telah dilakukan. f. Lakukan penyelidikan ditempat kejadian, lengkapi form laporan kecelakaan dan laporan penyelidikan tersebut dikirimkan ke HSE Departemen secepatnya. Dalam hal kecelakaan ringan : a. Pastikan bahwa semua korban telah mendapatkan perawatan medis, dan lakukan pengamanan yang diperlukan terhadap semua yang terlibat. b. Beritahukan kecelakaan tersebut kepada HSE Departemen dan pengawas langsung yang bersangkutan. c. Lengkapi form laporan penyelidikan kecelakaan dan kirimkan ke HSE Departemen. d. Lakukan tindakan perbaikan untuk mencegah agar kecelakaan tersebut tidak terjadi lagi. Dalam hal terjadi insiden/ near miss : a. Pastikan bahwa kegiatan ditempat kerja dimana terjadi insiden/ near miss dilakukan penghentian untuk mencegah agar tidak terjadi kecelakaan. b. Diskusikan dengan segera keadaan tersebut, ketahui jenis tindakan yang segera diperlukan dan selanjutnya lakukan tindakan - tindakan perbaikan. c. Buat laporan kejadian, sebutkan tindakan yang dilakukan sehingga keadaan tersebut benarbenar telah menjadi aman dan bicarakan dengan pengawas langsung.

Flammable liquids like petrol are not to be used as cleaning agents, use only approved cleaning solvents Store all flammable or combustible liquids and gases in a well-ventilated, cool place free from sources of ignition Do not remove or tamper with fire exthinguishers installed on equipment, vehicles, or in other locations. Access to fire fighting equipment must be kept free from obstacles that could delay emergency use. Familiars your self with the location and use of the project’s fire fighting equipment.Know the exit routes from buildings and work areas. Different types of exthinguishers are for different types of fires : a. Dry Chemical Powder (DCP) – These can be used on all fires but mainly for flammable liquid fires, and fires involving live electrical equipment b. Foam extinguisher – Use this on flammable liquid fuel type fires (petrol, oil, thinner, and solvent). DO NOT use on electrical fires. c. Carbon Dioxide – These can be used on all types of fires but are not particularly effective. They are best for use on electrical and electronic equipment because there is no residue and not a conductor of electicity. Exthinguishers are inspected monthly, serviced every six (6) month, and must be serviced or recharged immediately after every use Extinguish all matches, cigarettes, cigars and pipe tobacco before discarding. Storage of flammable substances on equipment or vehicles is prohibited unless designed for such use Before using open flame tools, make through inspection of the area for live sparks.

Biasakan diri anda dengan lokasi dan penggunaan alat pemadam.Ketahui jalur penyelamatan dari dalam gedung atau tempat kerja anda. Terdapat jenis alat pemadam yang berbeda untuk jenis kebakaran yang berbeda : a. Sebelum menggunakan perkakas dengan nyala api terbuka, periksalah secara seksama apakah disekitar tempat tersebut ada bahan yang mudah terbakar.

Clothing must be washed regularly 6. Uniform must not be worn as street clothes 7. Hands must be kept clean 8. Nails should be kept short, without nail polish 9. Cuts, scratches and other wounds must be covered with a waterproof bandage and changed regularly 10. People who are ill MUST NOT work. Symptoms or conditions such as diarrhea, vomiting, gastro-enteritis and bad coughs should be reported to managers or supervisors 11. It is against the law to smoke in areas where food is handled or prepared

XIII. KEBERSIHAN PERORANGAN 1.

Disamping aspek-aspek hygiene dari rambut yang tidak tertutup, ada bahaya besar dari lilitan mesin yang menyebabkan luka yang parah. Rambut yang panjang haruslah selalu diikat dan ditutup 4. Infeksi diusus, kulit dan hidung atau kerongkongan mestilah segera dilaporkan kepada petugas kesehatan sebelum berakibat pindah kepada orang lain. 5. Pakaian harus dicuci dengan teratur 6. Baju seragam tidak boleh dipakai untuk berjalan-jalan 7. Tangan harus dijaga kebersihannya 8. Kuku harus pendek dan tanpa cat kuku 9. Luka sayat, lecet dan luka lainnya harus ditutup dengan perban tahan air dan diganti dengan teratur 10. Orang yang sakit JANGAN bekerja Gejala atau kondisi seperti buang air, muntah, panas dalam perut, dan batuk menerus harus dilaporkan kepada manajer atau pengawas 11. Boleh merokok di area merokok


XIV. LOCK OUT / TAG OUT PROCEDURES The purpose of this procedures is ti give personal protection to an individual from the inadvertent starting or energizing of equipment while it is being worked on. Also to prevent the use of equipment, plant or machinery whilst in an unsafe condition. 1. Danger tag used by a person working on equipment to prevent starting or energizing of that equipment. Before tou start any job where the operating of a switch, valve, machine, etc.will put you in danger. You must place your “Danger” Tag and padlock on the main isolating switch, valve etc. 2. Absolute certainty is to be made that you have tagged the correct switch, valve etc 3. All danger tags must be in place before you commence work. On each tag you must print your name, date and time. 4. When two or more employees are working on the some job, each employee must attach their own “Danger” Tag and padlock on the main isolating switch or valve etc. 5. “Danger” Tags must be fastened securely so that they will not become detached. 6. The only person permitted to remove a danger tag is the person who places it in position. 7. “Out of service“ tag used to indicate a piece of equipment has been removed from service pending repair or replacement. 8. “Out of service” tags are not to be used in place of “Danger Tags” 9. An “ out of service” Tag is ONLY to be removed from a piece of equipment by the person affecting repairs or replacement or by a supervisor. When that person or supervisor is satisfied that the equipment is in a usable and safe condition

XIV. PROSEDUR LOCK OUT / TAG OUT Prosedur ini bertujuan untuk melindungi seseorang dari kecerobohan orang lain yang menghidupkan alat yang sedang diperbaiki, juga bertujuan untuk mencegah penggunaan alat, mesin yang tidak aman untuk digunakan. 1. Danger Tag digunakan /dipasang pada suatu alat untuk mencegah agar alat tersebut tidak dihidupkan. Sebelum mulai bekerja pada suatu alat dimana pengoperasian swich, valve mesin dll dapat membahayakan anda. Anda harus memasang danger tag dan kunci gembok pada swich utama isolator, valve utama dll. 2. Pastikan bahwa anda telah memasangnya ditempat yang benar. 3. Semua danger tag harus dipasang sebelum anda mulai bekerja, disetiap label anda harus menulis nama, tanggal, dan waktu. 4. Bila 2 orang atau lebih karyawan bekerja pada pekerjaan yang sama, masing2 karyawan harus memasang label danger tag dan kunci gembok di switch utama isolator atau valve utama dll. 5. Danger tag harus diikat dengan kuat supaya jangan terlepas. 6. Orang yang diijinkan melepas label danger tag adalah orang yang mempunyai atau memasangnya. 7. Out of service tag digunakan untuk menandai bahwa sebagian peralatannya / part telah diambil untuk diperbaiki. 8. Label out of service tidak boleh digunakan sebagai pengganti label danger tag. 9. Label out of service hanya boleh dilepaskan dari alat oleh orang yang memperbaikinya / menggantinya / supervisor. Jika supervisor tersebut sudah yakin bahwa alat itu sudah bisa digunakan dan dalam kondisi aman untuk dioperasikan.


XV.VEHICLE PROCEDURES 1.

Company vehicles will be regularly inspected and maintained by a qualified mechanic 2. Always operate any vehicle at a safe speed. This may be less than the area speed limit, if dusty condition or rain prevails (30 km/h) 3. Prohibited articles to be carried : a. Alcoholic drinks b. Any drugs unless prescribed and reported c. Any weapons including but not limited to knives,cutters,crossbows, catapults, swords, ammunitions and explosive devices d. Radioactive, corrosive, toxic, pressured container 4. When reverse parking the following steps are to be implemented : a. Ensure the area is clear, accessible and safe b. Check the mirrors constantly and reverse slowly c. Apply parking brake. 5. Vehicle must park at designated area at all work areas and stand by areas 6. When parking the vehicle the hand brake must be applied, the gearbox must be angaged and the engine must be turned off 7. Vehicles must be parked with a space of 2 meters between vehicles 8. Fueling or repairs must not be performed while the equipment is in use or the power on 9. Safety belt must be used 10. Do not ride in the bed of a truck containing materials which are not properly secured to prevent movement.

Radioaktif, bahan penyebab karat, bahan mengandung racun, baik dalam bentuk kemasan / bejana bertekanan. 4. Ketika memundurkan kendaraan ikuti langkah berikut : a. Pastikan daerahnya kosong, bisa dilalui dengan aman b. Selalu lihat kaca spion dan mundurlah dengan perlahan. c. Gunakan rem parkir 5. Mobil harus diparkir ditempat yang telah ditentukan disemua lokasi kerja dan tempat stand by 6. Ketika parker rem parker harus dipasang , masukkan gigi dan mesin harus dimatikan 7. Mobil harus diparkir dengan jarak minimum 2 meter antar kendaraan. 8. Pengisian bahan baker atau perbaikan2 tidak boleh dilaksanakan pada waktu alat sedang dipakai atau sedang jalan. 9. Sabuk pengaman harus dipakai. 10. Jangan naik dibak truk yang memuat barang2 yang tidak terikat dengan erat untuk mencegah agar barang tersebut tidak bergeser.

Setiap perancah harus diperiksa dan disetujui oleh pengawas yang bertanggung jawab sebelum dipergunakan untuk pertama kalinya dan juga setelah dirobah atau dipindahkan. Tidak ada apa yang disebut perancah sementara. Semua perancah harus dibuat dan dipelihara agar sesuai dengan pedoman2 yang telah ditetapkan Kayu untuk papan perancah tebalnya harus rata dan paling sedikit berukuran 2x10 inci, memenuhi mutu untuk perancah atau yang sama dengan itu. Papan perancah harus dilihat dan diperiksa tiap kali hendak dipakai. Papan2 perancah yang rusak harus segera dihancurkan. Tangga untuk naik turun harus disediakan pada tiap perancah. Perancah2 jangan dimuati terlalu berat. Bahan2 dinaikkan seperlunya saja. Perancah2 tidak boleh dimuati melebihi 0,25 dari kapasitas yang direncanakan untuknya. Tong2 besar, kotak2, tong2 kecil dan benda2 sejenis yang tidak stabil tidak boleh digunakan sebagai alas berdiri untuk bekerja atau menyangga perancah.


XVII. WELDING & BURNING OPERATIONS Welding & burning operations have a potential for personnel injuries and fires. When doing either, you must follow these precautions : 1. Before starting to burn or weld you must inspect your work area to ensure that sparks or molten metal won’t fall on combustible materials. If you can’t provide the necessary safe guards, check with your supervisor. 2. You must not weld or burn in a hazardous area without obtaining written authorization from the responsible authority. 3. You must be sure that suitable fire exthinguishing equipment is available in your work area. 4. You are responsible for maintaining your burning or welding equipment in safe operating condition. 5. When burning or welding, you must wear approved eye protection, with suitable filter lenses. 6. Keep all welding leads and burning hoses neat, tidy and safe 7. Never weld or burn barrels, thanks, piping or other systems which may have contained combustible or unknown products without first obtaining approval from your safety representative or other responsible authority. 8. When you arch – weld near other workmen they must be protected from the arc rays by non combustible screens or must wear adequate eye protection 9. The frames of all welding machines must be grounded. 10. Do not use matches to light torches. Spark igniters must be used. Torches must not be used to light smoking materials.

XVII. PEKERJAAN PEMBAKARAN DAN PENGELASAN Pekerjaan2 pembakaran dan pengelasan merupakan sumber kemungkinan terjadinya kecelakaan2 yang dapat menimbulkan luka2 dan kebakaran. Bila melakukan pekerjaan2 ini anda harus mengikuti petunjuk2 berikut ini : 1. Sebelum mulai membakar atau mengelas anda harus memeriksa tempat kerja anda untuk memastikan percikan2 api atau logam2 yang meleleh tidak akan jatuh pada benda2 yang mudah terbakar. Bila anda tidak dapat menyediakan alat2 pengaman yang diperlukan rundingkanlah dengan pengawas anda. 2. Jangan sekali-kali mengelas atau membakar ditempat yang berbahaya tanpa mendapat izin khusus, tertulis, dari petugas yang bertanggung jawab. 3. Anda harus memeriksa dengan baik apakah alat pemadam api yang cocok telah tersedia ditempat kerja anda. 4. Anda bertanggung jawab atas pemeliharaan alat las dan alat2 bakar agar alat2 tersebut selalu dalam keadaan baik untuk dipergunakan. 5. Pada waktu mengelas atau membakar anda harus memakai alat pelindung mata yang telah ditentukan untuk dipakai, dengan lensa2 penyaring yang cocok. 6. Atur agar semua kabel2 las dan selang2 pembakar rapi, teratur dan aman. 7. Jangan sekali2 mengelas atau membakar tong2, tangki2, pipa2, atau benda2 lainnya yang mungkin berisikan bahan yang mudah terbakar ataupun bahan2 yang tidak diketahui, tanpa izin terlebih dahulu dari petugas Keselamatan anda atau petugas lain yang bertanggung jawab. 8. Bila anda mengelas listrik didekat pekerja2 lain mereka harus dilindungi terhadap sinar2 lengkung listrik dengan layar2 yang tidak dapat terbakar atau dengan alat pelindung mata yang cukup memberi perlindungan. 9. Kerangka dari semua mesin2 las harus dihubungkan dengan tanah. 10. Jangan memakai korek api untuk menyalakan suluh las. Pakailah pemantik api.Suluh2 las tidak boleh dipakai untuk menyalakan benda2 berasap.

Hazardous substances, which by their nature or quantity constitute a hazard from explosion, fire, poisoning, gases, vapours or corrosive effect Material Safety Data Sheet (MSDS) is an information document which set out properties of the substance, precautions, first aid requirements, storage, and emergency response, so as the substance may be safely handled, stored and used at the work place. All employees must have acces to information regarding chemicals that are or may be used in the work place. Hazardous substances consist of material that includes gasoline, diesel fuel, solvents, some paints, acid, some chemicals and other products You must be aware as the presence formed as to the presence of these materials at your job site. MSDS must be kept on hazardous substances.Emergency procedures relating to these substances shall be made available to all employees All hazardous substances must be stored in the approved storage areas and these areas have signs to identify them. All personnel must watch out for spills and leaks and report them to supervisor Employees working with hazardous materials must be supervised.

Bahan berbahaya adalah bahan yang karena keadaan atau jumlahnya mempunyai bahaya terhadap ledakan, kebakaran, racun, gas, uap atau yang bersifat merusak Lembaran Data Keselamatan Bahan (MSDS) adalah dokumen informasi mengenai sifat2 suatu bahan, peringatan penggunaan, informasi bahaya, P3K, penyimpanan dan tindakan darurat agar bahan itu bisa ditangani, disimpan atau digunakan secara aman ditempat kerja. Semua karyawan harus mempunyai akses ke informasi yang berhubungan dengan bahan kimia yang ada atau mungkin dipakai ditempat kerja. Bahan2 berbahaya terdiri dari bensin, solar, cairan pelarut, cat, asam, beberapa bahan kimia dan produk lainnya. Anda harus mengetahui tentang keberadaan bahan berbahaya tersebut ditempat kerja anda. Semua bahan berbahaya harus tercantum dalam MSDS. Prosedur keadaan darurat untuk keadaan ini harus tersedia buat semua karyawan. Semua bahan berbahaya harud tersimpan ditempat penyimpanan yang telah ditentukan dan didaerah tersebut harus diberi tanda agar mudah dikenali. Semua karyawan harus waspada terhadap tumpahan dan kebocoran, laporkan hal itu ke supervisor anda. Karyawan yang sedang melakukan pekerjaan yang berhubungan dengan bahan2 berbahaya harus bekerja dibawah pengawasan.


XIX. OPERATING FORKLIFT’S Operation of forklifts should be limited to authorize operators only. These operators hold full responsibility in its maintenance and operation. To prevent accidents and to minimize possible hazards during the operation of forklifts, the following procedures should be followed : 1.

Forklifts should not be tempered with. Hence, the position of control levers remains intact, is it was when the forklift was received from the manufacturer 2. Operators should test forklifts before operations and maintenance work should be based on procedure recommended by manufacturer 3. if forklift is out of order and for that reason is unsafe to operate and needs repairs, its operation must be halted until it is completely fixed by appointed mechanics. 4. Do not drive forklift too close to a person standing in front of an object 5. No pillion rider is allowed in the vehicle, except when there is a space designated for assistant operators 6. Forklift should be parked on appointed area, and not in an alley or entry way. Parking in non designated places will disturb movements of other vehicles and goods 7. When forklift is left, the forks should be completely lowered, control levers made neutral, machine turned off and hand brake applied. Should forklift be parked on a sloping spot, be sure that wheels are properly wedged. When driving forklift be sure to keep a safe distance (three times the length of your vehicle) from other vehicles moving behing or before you 8. Operator should slow down his vehicles in the wet or slippery roads, and he should blow the whistle when approaching intersection and in other obstructed sight 9. If things lifted block his front view, operator should run forklift backwards 10. When climbing up and down slightly sloping terrain and other steep slopes, loaded forklift should be operated with the load lifted high and tilted

PENGOPERASIAN FORKLIFT Pengoperasian forklift harus dibatasi kepada operator yang berwenang saja, dimana yang bersangkutan memegang tanggung jawab utama atas keselamatan pengoperasian forklift tersebut. Untuk mencegah kecelakaan dan untuk mengurangi bahaya2 yang timbul dalam pengoperasian forklift, ketentuan berikut ini perlu diperhatikan : 1. Forklift tidak boleh dirubah sehingga posisi alat pengontrolnya akan berbeda dari keadaan semula ketika diterima dari pabrik pembuatannya. 2. Forklift harus diuji setiap hari oleh operator yang bertugas sebelum dioperasikan. Program pemeliharaan harus dilaksanakan sesuai recomendasi pembuatnya 3. Bila suatu waktu forklift rusak atau tidak aman dipakai 4. Forklift tidak boleh dikemudikan hingga dekat pada seseorang yang berdiri didepan suatu benda 5. Para penumpang tidak diperkenankan menumpang diatas forklift kecuali ada tempat yang khusus dirancang untuk pembantu operator 6. Memarkir forklift hanya ditempat yang ditentukan, jangan digang atau pintu masuk , jangan menghalangi peralatan atau barang. 7. Bila forklift ditinggalkan, garpunya harus benar2 diturunkan, alat control dinetralkan, mesin dimatikan dan pasang rem tangan. Bila forklift harus diparkir ditempat 8. Operator harus memperlambat kecepatan pada jalan basah atau licin dan harus membunyikan klakson pada semua persimpangan jalan dan lokasi2 lain dimana penglihatan terhalang 9. Bila muatan yang sedang diangkat menghalangi pandangan ke depan, operator harus mengemudikannya dengan cara mundur. 10. Bila akan mendaki atau menuruni jalur landai atau tanjakan2 lain, forklift yang bermuatan harus dikemudikan dengan muatan diangkat ke atas dan condong ke belakang

Forklifts weight capacity gauge should be provided, and this must be visible from the operators seat. Operator should be knowledgeable about capacities of various forklift 14. When operating electrically powered forklifts, operators should see to it that the battery cover is tied correctly and tightly 15. Do not apply back lever to brake electrically powered forklifts 16. Backward motion alarm should be provided at the back of all forklift to warn people when the vehicle is moving backward

Jangan menggunakan control mundur pada forklift listrik untuk mengerem 16. Alarm tanda mundur harus dipasang dibagian belakang dari semua forklift untuk memberi aba-aba pada orang lain apabila forklift itu bergerak mundur.


XX. MANUAL HANDLING Following these simple steps will ensure that you minimise the risk of injury when performing Manual Handling tasks. 1. Assess the size and shape of the load before attempting to handle. Make allowances for the bulk of the load as well as the weight and get help if you need it. 2. Consider the best path to take when handling the load. The shortest path may not always be the best way. If there are many obstructions in the way, perhaps an alternative path should be considered. 3. Assume a well-balanced position facing the direction you intend to move the load. The feet should be parted with one foot alongside the object to be lifted and one behind. The feet comfortably spread gives greater stability and the rear foot is in position for the upward thrust of the lift. 4. The hold must be secure and comfortable. Use palms of hands and not finger tips. For objects such as boxes, hold diagonally opposite corners. Move in close to the load with arms and elbows tucked in. 5. Bend at the knees and keep the back in its natural curved position. But remember that “straight” doesn’t mean “vertical”. A straight back keeps the spine, back muscles and body organs in correct alignment. 6. Keep the head raised and chin in so the neck and head continue the straight back line. 7. Leg muscles are stronger than back muscles. Use them to lift the load. Do not jerk or strain, use a smooth action. 8. Start the lift with a thrust from the rear foot and follow through with the body when possible. Use of body weight helps to propel the load, reduces fatigue and strain on the arms. 9. Mechanical assistance and / or team lifting arrangements should be provided for weights above 16 kg.

Satu kaki harus disamping atau sepanjang object yang akan diangkat dan satunya lagi dibelakang.Kaki akan nyaman dan memberikan stabilitas lebih besar.Sedangkan kaki belakang adalah sebagai daya dorong untuk menaikkan / mengangkat. 4. Pegangan harus nyaman.Gunakan telapak tangan & tidak memegang Ujung / sudut.Untuk object seperti kotak pegang kebalikan sudut Diagonal. Pindah didekat beban dengan lengan dan siku memasukan ke dalam. 5. Menekuk lutut dan menjaga punggung pada posisi curve alami.Tapi ingat bahwa lurus tidak berarti vertical. Punggung lurus akan memelihara tulang belakang, mengembalikan otot dan organ pada posisi yang benar. 6. Jaga ketinggian kepala dan dagu sehingga kepala dan leher tetap segaris dengan punggung, 7. Otot kaki lebih kuat daripada otot punggung,biasakan mereka mengangkat beban seperti itu.Jangan menghentak atau tegang, gunakan tindakan yang lembut. 8. Pada saat mengangkat gunakan daya dorong kaki belakang dan dilanjutkan dengan badan bila memungkinkan.Penggunaan berat badan membantu menggerakkan beban itu , mengurangi kelelahan dan ketegangan pada lengan 9. Bantuan mekanik atau pengaturan regu pengangkat harus ada untuk beban diatas 16 kg.

If you have a visitor with you, it is your responsibility to ensure that she/he complies with this procedure. 5. Ladies should not wear high-heeled shoes and stocking on evacuation. 6. Handicapped or pregnant people should ask assistance from Fire Wardens. 7. Gather at the Designated Muster / Assembly Point (refer to Emergency Exit Guidelines located at each room/cubicle). 8. Do not drive your car out of the car parks.

Jika anda mempunyai tamu dengan anda, Itu adalah tanggung jawab anda untuk memastikan bahwa tamu tersebut mematuhi prosedur ini. 5. Bagi karyawati mestinya tidak memakai kaus kaki dan sepatu yang tinggi pada saat pengungsian. 6. Orang yang hamil atau orang cacat jasmani perlu minta bantuan / bertanya kepada fire warden. 7. FIRST AIDERS The roles and responsibilities of First Aiders: 1. To give immediate care to a person who has been injured or suddenly taken ill if medical assistance is not available or is delayed. 2. To avoid errors those are frequently made by untrained persons in helping injured or suddenly ill person. In the event of illness or injury of an employee or visitor, one of the First Aiders and respective supervisor will be contacted immediately by the person who happens to see it. BOMB THREAT PROCEDURE Modes of bomb threat could be a bomb threat call (message), a suspicious package, or a letter / package bomb.

1. Be calm and do not panic. 2. Call:  extension 103 if you are within PT.ENVITECH PERKASA offices (Building Security Post) 3. Management will make an announcement to all employees on the threat. Employees are requested to follow further instructions from Management. 4. Conduct a quick inspection of your immediate work area observing any suspicious object. Do not touch or try to move the object. 5. ON RECEIPT OF A BOMB THREAT CALL a. When you receive a bomb threat call, employee shall explore information as much as possible . b. Engage the caller in conversation in an attempt to discover as much information as possible. c. When the call is finished, contact extension 103 immediately. d. The Telephone Operator shall contact the Chief Security or Emergency Commander or Envitech Management ERT to inform the bomb threat.

PROSEDUR TERHADAP ANCAMAN BOM Model ancaman pengeboman bisa melalui telepon / pesan, suatu paket yang mencurigakan, atau suatu surat / paket bom. 1. Tenang dan jangan panik. 2. Hubungi : extention 103 jika anda ada di kantor PT. Envitech Perkasa 3. Manajemen akan membuat suatu pengumuman untuk semua karyawan mengenai ancaman tersebut. Karyawan diminta untuk mengikuti perintah lebih lanjut dari Manajemen. 4. Melakukan suatu pemeriksaan dan observasi yang cepat dari area kantor terhadap obyek yang mencurigakan. Jangan menyentuh atau mencoba untuk memindahkan obyek tersebut. 5. PADA SAAT MENERIMA SUATU ANCAMAN PENGEBOMAN a. Ketika anda menerima suatu ancaman pengeboman, karyawan akan menyelidiki informasi sebanyak mungkin. b. Mencoba melibatkan si penelepon dalam percakapan untuk memperoleh informasi sebanyak mungkin. c. Ketika ancaman sudah selesai, cepat menghubungi ext 103 . d. Operator Telepon akan menghubungi pimpinan Security/Keamanan atau Pemimpin keadaan darurat atau Envitech ERT Manajemen untuk menginformasikan ancaman pengeboman tersebut.

e. Jangan mencoba untuk menemukan bom tersebut, tetapi mempersiapkan evakuasi dari gedung (sesuai dengan Procedure evakuasi).

4.2.1 Internal Audit Preparation

Before auditing, the team prepares for auditing stuff such as: Check List, Audit Schedule, Stationery The – soon to be audited – department will receive a letter notifying the auditors’ future coming and this letter is sent by HSE Department

4.2.2. Internal Audit Implementation 1. Audit activities will begin with an opening meeting to explain the presence of the auditors and to set up auditing schedule analysis. 2. Audit activities will be carried out based on the auditing schedule that has been approved by the auditee. These include activities such as: A. Examination of related documents B. Interview with the auditee or workers C. On – field inspection visit. 3. In the closing meeting the auditors will explain the discrepancies found in he audite. If the auditee agrees with the findings, the auditors will make a Non-Conformance record (NCR) as in improper form of HSE 4. The Non-Conformance Report (NCR) made by the auditor will be sent to the auditee within three (3) days. The auditee will fill in the proposal for improvement /prevention in the report and set a new schedule. This report will be approved by the related Department Managers. 5. Verification of the improvement/prevention effectiveness is carried out in the followup audit by the personnel of HSE Department. 6. If the improvements have been implemented and carried out well, the NCR report is closed. The report is verified by the personnel of HSE Department who are appointed and signed by the HSE Manager as the management representative. 7. If improvements are not conducted until the approved date, the new NCR is released and the related department should set a new schedule. 8. After a new NCR closes, it is signed by HSE Manager. 9. The Operation Manager as recommended by HSE Manager, will set the follow-up audit for the department whose audit result is not yet satisfied.

Walk around your machine before you mount it 3. Make certain no one is working on, under, or close to the machine 4. Inspect your machine for potential hazards. Fill out pre-start check list 5. V. DETAIL PROCEDURES The appropriate solid waste management shall be started with pollution prevention and directed to the waste disposal; the change and reducing the operational practices which resulted the disposal into land/earth, air and water.

5.1.

Preparation

5.1.1. Provide waste container prior the work is started by envitech or client 5.1.2. The Waste Container should be clean and completed with the cover 5.1.3. The Waste Container shall be water proof, not easily corrosion. 5.1.4. The volume and quantity of Waste Container should be adjusted with the product waste resulted at each operation unit.

5.1.5. Be available at place which producing the solid waste 5.2.

Separation 5.2.1. Organic waste and anorganic waste should be separated before they are disposed into the determined Final Waste Disposal. 5.2.2. Organic Waste shall be degradable waste and degraded by microorganism such as: food left over, kitchen waste, vegetables, paper and others,

the container used to patch the waste should be in

green color. 5.2.3. Anorganic Waste shall be non degradable waste which is not easily degraded by micro organism; to destroy such waste will need special handling such as: metal, tin, broken plate glass, bottle and so on; the container to patch them should be in red color. 5.2.4 Dangerous and Poisonous Waste B3 shall be

Transporting 5.4.1. The collected waste in the temporary waste collection/container is transported using a special vehicle for waste transportation. 5.4.2. The garbage truck shall be covered to protect from cross contamination through the air. 5.4.3. The transportation is conducted once a day.

5.5.

Waste disposal and destruction

Keep hand free (Do not take a goods or bags when entering to the speed boats) 3. All passangers must always wear life jacket in the speed boats 4. Follow instructions from master / driver 5. No smoking is allowed during refuling a speed boat 6. After arrived, please wear life jacket till on the platform

m) Fire extinguisher should be available (Carbon dioxide, dry chemical or water spray) APAR (Alat Pemadam Api Ringan) harus ada (CO2, dry chemical atau water spray) n) Monitoring for methan gas is required before starting the job, please use the gas detector Monitoring gas methan dipersyaratkan sebelum pekerjaan dimulai , mohon menggunakan gas detector o) Safety harness or safety belt is mandatory required when working at height . Safety harness atau safety belt dipersyaratkan apabila bekerja diketinggian p) Fire alarm, Fire Hydrant, Fire hose should be available Fire alarm, Fire Hydrant, Fire hose harus ada disekitar area reactor
