REMONTA Para hacer la remonta se necesita hacer un modelo que reproduzca reproduzca los dientes naturales y que permita la remoción y asentamiento de la dentadura parcial terminada para poder perfeccionar la oclusión en el articulador. Para confeccionar este modelo, se coloca la dentadura en la oca del paciente y se selecciona una cueta de stoc! que cura los dientes naturales y la dentadura , se prepara al"inato y se toma una impresión de todo el con#unto$ al retirar la impresión de la oca la dentadura parcial dee salir #unto con la impresión, impresión, si esto no ocurriera, ocurriera, se retira la dentadura de la oca y se coloca en su posición correcta dentro de la impresión del interior del cual se han retirado pre%iamente con la ayuda de una ti#era &na y a&lada, todos los pedazos de al"inato lire que puedan interferir en la correcta reposición de la dentadura dentro de la impresión. se 'uye una capa del"ada de cera sore las porciones retenti%as de los retenedores y porciones %erticales de la estructura met(lica. )on papel de ser%illeta h*medo se loquean los (n"ulos retenti%os de la ase de la dentadura de#ando siempre lires lires los ordes de la dentadura misma para facilitar su remoción y su reposición. +e prepara yeso piedra y se hace el modelo sin %irar la impresión para e%itar que el yeso 'uya por dea#o de la dentadura. na %ez fra"uado el yeso se retira la impresión y se remue%e con cuidado la dentadura del modelo. +i los dientes de yeso son rotos en este proceso o al separar la impresión, ellos pueden ser ados en su sitio con cemente de o-ifosfato de zinc. na %ez conse"uido el modelo para el remonta#e, se eliminan las uru#as u otras discrepancias de las super&cies oclusales de los dientes de yeso para "arant "arantiza izarr una uena uena relac relación ión entre entre los model modelos os superi superior or e infer inferior ior en el articulador. +e coloca de nue%o la dentadura en la oca del paciente y se hace un re"istro en relación cntrica de modo que los puntos que hacen el primer contacto queden lo m(s cerca posile entre s/ sin lle"ar a tocarse. El material de re"istro re"istro m(s aconse aconse#a #ale le para para estos estos casos casos es el yeso yeso para para impre impresi sione ones. s. na %ez tomado el re"istro, se eliminan los e-cesos del yeso para impresiones con un cuchillo a&lado y se relacionan relacionan los modelos y el re"istro ase"ur(ndolo ase"ur(ndolo con cera pe"a#osa, lue"o se remonta el modelo al articulador. Para hacer la remonta, "eneralmente no es necesario usar el arco facial porque se usa el modelo anta"onista que se empleó para articular los dientes, el mismo que dee conser%arse en el articulador. el arco facial para reorientar los modelos en el articulador. 0espus de fra"uado el yeso de la remonta se retira el re"istro interoclusal y se cierra el articulador hasta que se produzca el primer contacto entre los dientes opuestos o ases opuestas. +i el monta#e es correcto, el primer contacto estar( en el mismo sitio que el %isto en la oca, si el monta#e no es similar a aquel %isto en la oca, se hace un nue%o re"istro y remonta. +i hay errores, estos se %en m(s "randes en el articulador que en l aoca deido a que en esta los te#idos desplazales disimulan parte de la discrepancia e-istente. El remonta#e de los modelos dee ser e-acto y por consi"uiente, es frecuente tener que repetir el procedimiento si se quiere resultados correctos. na %ez remontada la dentadura en el articulador se procede al a#uste oclusal si"uiendo un orden que comienza con el a#uste de los contactos en oclusión cntrica. A1+TE 0E 2O+ )ONTA)TO+ 0ENTAR3O EN O)2+3ON )4NTR3)A Para marcar los contactos iniciales en oclusión cntrica se coloca papel de articular entre los dientes y se "olpean sua%emente entre si con el articulador en su posición cntrica. Es aconse#ale mantener el papel de articular en amos lados parra que el da5o a los dientes de yeso se manten"a al m/nimo al hacer contacto con sus anta"onistas. El (rea de contacto inicial aparece "eneralmente con un c/rculo del color del papel de articular empleado y un centro lanco. +e des"astan estas (reas de los dientes arti&ciales remo%iendo las estructuras dentarias con piedras montadas redondas o con fresas redondas 67 o 689. 2as fosas o las %ertientes de las fosas de los dientes posteriores son las (reas que "eneralmente se des"astan primero. +i el error de la oclusión es un marcado contacto a ni%el de un molar, se contin*a el des"aste hasta que los premolares entren en contacto. +e dee mantener una atención constante en el sorepase %ertical de los diente anteriores para determinar cu(ndo se ha hecho su&ciente des"aste de los dientes posteriores. +i el error de la oclusión est( en un fuerte contacto entre los premolares o los dientes anteriores, se des"asta las estructuras dentarias hasta que las molares entren en contacto. Este tipo de fuerte contacto anterior, es "eneralmente m(s dif/cil de corre"ir en un fuerte contacto molar porque requiere m(s des"aste de los dientes anteriores para cerrar un espacio posterior. Puede ser necesario rearticular los dientes posteriores en oclusión para corre"ir fuertes contactos dentarios anteriores. )uando los dientes parecen estar "olpeando uniformemente en amos lados, se coloca entre los dientes un pedazo de papel &no que %iene entre las ho#as del papel de articular. El papel dee ofrecer resistencia al tirar de el en el momento que est( entre los molares y premolares cuando se ha conse"uido un contacto uniforme en amos lados.
+e retira la dentadura del articulador y se lle%a a la oca del paciente y se le pide que cierre en relación cntrica. +i se le pide que cierre en relación cntrica. +i el monta#e fue correcto, todos los dientes posteriores deen contactar uniformemente en el punto de contacto inicial, e-actamente i"ual que en el articulador. +i e-iste una diferencia entre el monta#e y la oca, se dee hacer un nue%o re"istro interoclusal en relación cntrica y un remonta#e en el articulador. A1+TE 0E 2O+ )ONTA)TO+ 0ENTAR3O+ EN O)2+3ON E:)ENTR3)A +e hacen en el paciente los re"istros interoclusales e-centricos despus de haer practicado, haciendo que este cierre hacia las %arias posiciones e-cntricas. )on estos re"istros de a#ustan las an"ulaciones condilares del articulador. +e mue%e el articulador hacia la posición de traa#o con el papel de articular entre los dientes en amos lados. +i huiera &rmes contactos en el lado de traa#o, se reducen los planos inclinados lin"uales de las c*spides ucales superiores y los planos inclinados ucales de las c*spides lin"uales inferiores. +i hay fuertes contactos en el lado de alance se hace un surco para el pasa#e de las c*spides palatinas superiores a tra%s de la super&cie oclusal de las piezas posteriores inferiores. 2ue"o se haer hecho los a#ustes indicados en los mo%imientos de lateralidad se mue%e el articulador hacia la posición protrusi%a. +i huiera fuertes contactos en los dientes arti&ciales posteriores, se hace un camino para las c*spides palatinas superiores a tra%s del reorde mar"inal distal inferior. 2os planos inclinados lin"uales de las c*spides ucales de los premolares superiores inter&eren a menudo con el mo%imiento protrusi%o. +e reducen cuidadosamente estos planos inclinados para no interferir con los contactos dentarios en oclusión cntrica o en el lado de traa#o. 2ue"o se mue%e el articulador a tra%s de los l/mites normales de función de los dientes para localizar cualquier interferencia no re"istrada en los pasos pre%ios. A menudo la "u/a de los dientes naturales remanentes e%ita los contactos de los dientes arti&ciales en las posiciones que no sean la oclusión cntrica. na %ez hecho el a#uste oclusal los dientes posteriores pueden haer perdido su anatom/a oclusal por el des"aste, si este es el caso, se trazan con un l(piz los surcos oclusales de los dientes posteriores y lue"o, con la ayuda de una fresa cil/ndrica, se profundizan para recuperar la anatom/a oclusal y crear las %/as de escape para hacer m(s e&ciente la función masticatoria.
0espus que se ha estalecido un contacto simultaneo y uniforme entre los dientes en las %arias posiciones de la mand/ula, se repite el uso de la pasta indicadora de precisión. En este momento no se usa la presión di"ital si no la precisión de %arios contactos oclusales para poner en e%idencia cualquier e-ceso de presión. ;inalmente, se pulen los dientes des"astados cuidando de no alterar su anatom/a oclusal. )ONTRO2 0E 2A E+TET3)A. )ualquier a#uste para me#orar la apariencia esttica de la dentadura dee hacerse despus que se han hecho todos los a#uste pre%iamente mencionados para que la prótesis luzca mas esttica puede ser necesario acortar dientes, modi&car su contorno facial, producir los espacios interpro-imales, caracterizar los ordes incisales o la punta de al"unas c*spides, modi&car el talón de los dientes arti&ciales anteriores, etc. 2a realización de los procedimientos descritos y de cualquier a#uste que puede ser peculiar descritos y de cualquier a#uste que puede ser peculiar para una prótesis determinada dee conducir a una dentadura funcional y confortale. 3NTR))3ONE+ A2 PA)3ENTE <E RE)3=E NA 0ENTA0RA NE>A na %ez instalada en oca la dentadura nue%a no se despide al paciente sin antes darle las instrucciones sore las di&cultades iniciales de una dentadura remo%ile y como solucionar los prolemas que se presentaran, con proailidad muy "rande, el paciente tamin dee ser instruido sore la responsailidad que le compete para el cuidado de la prótesis y la solución de los incon%enientes iniciales, se le dee dar la información que prolon"ue la %ida *til de su dentadura y de las estructuras orales de soporte.
