AFFIDAVIT OF GUARDIANSHIP I, RANDY CABANAG TAN, of legal age, married, and resident of Brgy. Poblacion, Sibulan, Negros Oriental, Philippines, after having been sworn to in accordance with law, do hereby depose and state that: 1. I am the lawful husband of Maria Elena D. Deposoy, who is the elder sister of Ms. Rosita Deposoy; 2. Ms. Rosita Deposoy, who is now working in Japan as Overseas Filipino Worker (OFW), has left in my custody her daughter, namely Angela Jolie Deposoy; 3. My niece Angela Jolie Deposoy is now under my care and living with me, being the nearest kin to the family; 4. I hereby declare myself as the de facto guardian of my niece, Angela Jolie Deposoy; and 5. I am freely executing this affidavit to attest to the truth of the foregoing facts which I have personal knowledge of and also for the purpose of securing the duly authenticated birth certificate with the Philippine Statistics Authority (PSA), and for whatever legal purpose this may serve.
