Instructions to candidates The examination paper is in FOUR sections. All sections should be attempted. The time allowed is two hours. There is no additional time allowance for reading the question paper. There are a maximum of 100 marks available. In order to pass this examination you must achieve the pass mark of 70 and you must achieve at least 40% in each of the four sections. Your answers must be recorded on the sheets that are supplied. Additional loose sheets will NOT be accepted. The phrase “18001” always refers to OHSAS 18001:2007. The word “clause” refers to a paragraph or section of OHSAS 18001:2007 that is identified by a number such as 4.4.7 etc. This examination is open book. Section 1 is worth 10 marks It comprises 5 very short-answer questions that test your general knowledge of the subject matter covered throughout the course. Section 2 is worth 20 marks It comprises 4 short-answer questions that test your knowledge of ISO 19011 auditing techniques. Section 3 is worth 30 marks It comprises 2 questions (15 marks per question) that require a detailed written answer that will normally fill the space provided. Section 3 questions relate to auditing OH&S management systems. Section 4 is worth 40 marks It comprises audit situations which are designed to assess what actions you would take as an auditor. Section 4 questions are designed to assess your ability as an auditor.

As the third party auditor preparing for the first surveillance audit since the incident, identify at least four key issues for evaluation during the audit and at least six audit trails to be followed. Section 4: worth 40 marks Audit Scenario
During an audit of a large construction organization with a head office, two regional offices and 6 major construction projects underway, the lead auditor requests details of the organization’s OH&S objectives. The H&S Director says that he has responsibility for one key objective: To reduce the amount of lost time incidents with a target of a 10% reduction each year for the next 3 years. The director has established a series of initiatives to reduce the amount of incidents. These initiatives were supported by a staff training programme. The initiatives related to: working at heights, fire safety and road safety for the motorway maintenance staff. The lead auditor then established that the target was set against incidents that occurred in 2003/4 but the director was unable to show data from this period that indicated the type or cause of each incident. The audit team proceeded with the audit and asked operational staff about the objective and were given a positive response indicating that all those who were questioned had been trained and were aware of the initiatives currently in place. The process for recording and investigating incidents and near misses was reviewed and found to be satisfactory, although, the audit team found that in recent years a large amount of the lost time incidents were due to manual handling activities and that tasks that required manual handling activities were considered very high risk on risk assessment forms. It was also noted that these types of incidents had also been increasing over recent years. There were no records to indicate whether the organization had considered any alternative methods of handling heavy loads.
